Magnetresonanztomographische Darstellung der Liquorpulsbewegung bei Hydrozephalus communicans vor und nach Shuntanlage

Zusammenfassung

16 Patienten mit kommunizierendem Hydrozephalus sowie 5 gesunde Probanden wurden zur Darstellung des pulsatilen Liquorflußmusters mit einer flußsensitiven 2D-FLASH-Sequenz untersucht. Nach Anlage eines ventrikuloperitonealen-atrialen Shunts wurden die Patienten mehrmals kernspintomographisch kontrolliert, um eventuelle postoperative Änderungen der Flußdynamik nachzuweisen. Am häufigsten (n = 12) fand sich ein präoperativ hyperdynames Liquorpulsationsmuster, welches sich nach Shuntanlage im Sinne einer Reduzierung besserte (n = 8). Der Reduktion des pulsatilen Flußmusters stand in diesen Fällen eine klinische Befundbesserung gegenüber. Mit der vorliegenden Methode scheint eine klinische Entscheidungshilfe zur Frage einer eventuellen Shuntanlage bei Patienten mit Hydrocephalus communicans möglich.

Summary

16 patients with hydrocephalus communicans and 5 healthy volunteers were examined to demonstrate the pattern of the pulsatile CSF flow. After implantation of a CSF shunt system the same patients were examined again to show the influence of the shunt on the CSF pulsations. We used a flow-sensitised, cardiac-gated 2D FLASH sequence and analysed the phase and magnitude images. It could be shown that most patients (n = 12) had a hyperdynamic pulsatile flow preoperatively. After shunt implantation the pulsatile CSF motion and the clinical symptoms were improved in 8 of these patients. MM of pulsatile CSF flow movement seems to be a helpful noninvasive tool to estimate the prognosis of a shunt implantation in patients with hydrocephalus communicans.
