Abstract

Purpose: Extracorporal shock wave therapy is a controversial discussed treatment option in tennis elbow. Different studies revealed controversial results regarding the clinical benefit of 1 000 versus 2 000 impulses of constantly low energyfluxdensity.

Material and Methods: Twenty patients were treated with low energetic shock wave therapy for symptomatic tennis elbow in a prospective, randomized, single-blind pilot study. Symptoms were evident for at least 6 months. 2 different conservative therapies failed before assignment to the study. The population was randomised into two groups of equal numbers. All patients passed inclusion criterion. The shock wave application was performed three times in weekly intervals. Group one was treated with 1 000, group two with 2 000 impulses of constantly low energetic energyfluxdensity. Follow up examinations were performed after six and twelve weeks according to a standardised questionnaire.

Results: Overall, patients improved in function and pain after therapy. Subjective satisfaction was evident in both groups. Overall, group one obtained better results. Undesired effects were not observed. The population is too small to reveal statistical significant results.

Conclusion: Extracorporal shock wave therapy was helpful in the treatment of lateral epicondylitis. The application of 1 000 impulses was well tolerated by the patients and the procedure is quick.

Zusammenfassung

Fragestellung: Extrakorporale Stoßwellentherapie ist ein kontrovers diskutiertes Therapieverfahren in der Behandlung des Tennisellenbogens. Verschiedene Studien zeigen widersprüchliche Ergebnisse betreffend klinische Verbesserung zwischen 1 000 und 2 000 Impulsen mit konstant niederenergetischer Stoßwellentherapie.

Material und Methodik: Zwanzig Patienten mit symptomatischem Tennisellenbogen wurden mit niederenergetischer Stoßwellentherapie im Rahmen einer randomisierten, einfach verblindeten Pilotstudie behandelt. Die Beschwerdedauer bestand länger als 6 Monate. Zwei unterschiedliche konservative Therapieverfahren zeigten keine Wirkung. Die Population wurde in zwei Gruppen gleicher Anzahl an Probanden randomisiert. Alle Patienten bestanden die Einschlusskriterien. Die Stoßwellentherapie erfolgte in wöchentlichen Abständen. Gruppe eins erhielt 1 000, Gruppe zwei 2 000 Impulse im konstant niederenergetischen Bereich. Nachuntersuchungen erfolgten nach 6 und 12 Wochen mit einem vorgefertigten Fragebogen.

Ergebnisse: Insgesamt verbesserten sich alle Patienten in Bezug auf Schmerz und Funktion. Subjektive Zufriedenheit wurde in beiden Gruppen festgestellt. In der Summe erzielte Gruppe eins bessere Ergebnisse. Die Population war zu klein um signifikante Resultate zu erzielen.

Schlussfolgerung: Extrakorporale Stoßwellentherapie war hilfreich in der Behandlung des Tennisellenbogens. Die Behandlung mit 1 000 Impulsen wurde gut vertragen und sich als weniger zeitaufwenidges Verfahren.
