Zusammenfassung
Die Kombination einer gaumengestützten GNE-Apparatur mit der Pendelapparatur ist eine
extrem effiziente Apparatur für die gleichzeitige transversale und sagittale Expansion
des Oberkiefers bzw. des oberen Zahnbogens. Mit dieser Apparatur können in einem Behandlungsschritt
ohne größere Compliance-Anforderungen eine transversale Unterentwicklung des Oberkiefers
und eine erhebliche Vorwanderung der Molaren korrigiert werden. Gefolgt von einer
vollprogrammierten Bracketapparatur können mit diesem Behandlungsprotokoll zuverlässig
durchschnittliche aktive Behandlungszeiten von etwa 20 Monaten erreicht werden. Dies
liegt deutlich unter den in der Literatur angegebenen Behandlungszeiten. Das Behandlungsprotokoll
empfiehlt sich daher aus Gründen der Zeiteffizienz, ist jedoch wegen relativ hoher
ärztlicher Arbeitszeit nicht in gleicher Weise kosteneffizient. Wenn Kosteneffizienz
im Vordergrund steht, ist die Extraktion von Prämolaren ohne Zweifel überlegen. Mit
Rücksicht auf
die faziale Ästhetik wurde hier jedoch dem aufwendigeren Behandlungsprotokoll der
Vorzug gegeben.
Abstract
The combination of a tissue-borne RME-appliance with a pendulum appliance is an extremely
efficient appliance for simultaneous transverse and sagittal expansion of the upper
jaw and upper dental arch, respectively. By this appliance both maxillary transverse
deficiency and mesially migrated or tilted upper molars can be corrected in one step
and without special compliance needs. Followed by a fully programmed fixed appliance,
an average active treatment duration of 20 months can be achieved reliably, which
compares favourably to active treatment duration reported in the literature. The treatment
protocol lends itself for its excellent time efficiency. However, it is not cost efficient
to the same degree because of extended doctor working time required. If cost efficiency
is a matter of concern, premolar extraction will be favourable without doubt. With
regard to facial aesthetics the more complex treatment protocol was favoured in the
case presented.
