Die kieferorthopädische Behandlung von fehlenden mittleren und / oder seitlichen Schneidezähnen im Oberkiefer bei Nichtanlagen oder nach traumatischem Verlust findet in der Regel im jugendlichen Alter der Patienten statt. Deshalb verbleiben meist auch noch einige Jahre an Restwachstum der Kiefer und des Gesichtsschädels. Sofern eine Lückenöffnung indiziert ist und eine implantologische Versorgung geplant ist, treten häufiger klinische Probleme auf. Der größte Nachteil ist die lebenslange Abhängigkeit von einer festsitzenden prothetischen Versorgung in einem Bereich des Mundes mit besonderer ästhetischer Relevanz. Der implantologisch-prothetische Ersatz eines Einzelzahns im Frontzahnbereich des Oberkiefers ist eine besondere Herausforderung. Nicht nur, dass die implantatgestützte Krone in ihrer Form, Farbgebung und Kontur an die Nachbarzähne angepasst werden muss, sondern auch der Verlauf der marginalen Gingiva sollte sich hinsichtlich Symmetrie harmonisch an die angrenzenden Weichgewebe anpassen. Auch wenn ein Implantat unter dem Gesichtspunkt der Osteo- oder Osseointegration erfolgreich verlaufen ist und ein gutes funktionelles Ergebnis erzielt wurde, so kann das ästhetische Resultat mittel- und langfristig hinter den Erwartungen zurück bleiben.

[Abb. 1] zeigt klinische Bilder einer Patientin nach orthodontischer Lückenöffnung für einen nicht angelegten seitlichen Schneidezahn, 6 Jahre nach implantologisch-prothetischer Versorgung. Deutlich erkennbar ist eine ausgeprägte Rezession der marginalen Gingiva mit einem Durchscheinen des Kronenrandes aus Metall. Mit 24 Jahren hat aber diese Patientin sicherlich noch 50 oder 60 Jahre ihres Lebens vor sich!

Eine labiale und interdentale Rezession der Gingiva ist deutlich erkennbar und der Rand der Metallkeramikkrone sorgt für ein unästhetisches Aussehen.
