Zusammenfassung

Hintergrund: Zu den am häufigsten angewandten pflanzlichen Präparaten zur Therapie von Ein- und Durchschlafstörungen gehören Extrakte der Baldrianwurzel. Allerdings konnte bislang noch keine akute Wirkung eines Baldrianextraktes nachgewiesen werden. Deshalb wurde in dieser Studie, die nach GCP durchgeführt wurde, versucht, die Wirksamkeit von Baldrian-Dragees (Nervenruh Baldrian Forte; 600 mg Trockenextrakt, Verhältnis Droge zu Extrakt 3-6:1; Auszugsmittel 70 % Alkohol) mithilfe einer neuen, validierten Schlafanalyse nachzuweisen.

Methoden: Insgesamt wurden 36 Personen im Alter von 35 bis 70 Jahren mit Ein- und/oder Durchschlafstörungen in die Studie eingeschlossen. Die Auswahl erfolgte über die Erfassung und Auswertung der Symptomatik mithilfe des Schlaffragebogens SF-B. Alle »Schlechtschläfer« wurden in der ersten Nacht an die Verhältnisse im Schlaflabor adaptiert. In der zweiten Nacht erhielten sie eine Stunde vor dem Zubettgehen ein Dragee Placebo (n=18) oder Verum (n=18) in randomisierter, doppelblinder Verschlüsselung. Die konfirmatorische Per-Protokoll-Auswertung erfolgte mithilfe des validierten spektralen Frequenzindexes (SFx). Anhand dieses Parameters wurde der Schlaf in sukzessive Bereiche von Schlaftiefen unterteilt; für jeden Schlaftiefenbereich wurde dann die Verweildauer in Minuten bestimmt.

Ergebnisse: Die mit Nervenruh Baldrian Forte behandelte Gruppe zeigte im Vergleich zu Placebo generell eine höhere Verweildauer in den Schlaftiefenbereichen unterhalb von 77 % bis unterhalb von 62 %, also in Richtung normaler Schlaf. Die Irrtumswahrscheinlichkeit unterhalb von 77 % und unterhalb von 74 % betrug p<0,07 bzw. p<0,08. Darüber hinaus wurden in der Baldriangruppe deutlich weniger Bewegungsereignisse registriert (p<0,06). Die Verträglichkeit des Verums war sehr gut.

Schlussfolgerung: Mithilfe der neu entwickelten Methode der Bestimmung der Schlaftiefenzeit, basierend auf dem spektralen Frequenzindex (SFx), ist es zum ersten Mal gelungen, eine akute positive Wirkung eines Baldrianextraktes auf Ein- und Durchschlafstörungen messtechnisch quantitativ und objektiv zu erfassen.

Summary

Background: Extracts of valerian root belong to the most frequently prescribed herbal preparations for therapy of sleep disorders. But until now, no effects of a single dose had been established. Therefore an attempt is made in this study, which was performed according to GCP criteria, to verify the effect of valerian dragées (pharmaceutical preparation: Nervenruh Baldrian Forte; 600 mg dry extract, drug-extract ratio: 3-6:1, extraction medium: 70 % ethanol) by means of a new validated sleep analysis.

Methods: A group of 36 volunteers aged 35 to 70 years suffering from disorders of not falling asleep or/and not sleeping through the night was included into the study. The inclusion criteria were formulated according to a well known validated sleep score (SF-B; Collegium Internationale Psychiatriae Scalarum). All „bad sleepers” were adapted to the conditions in the sleep lab during the first night. At the beginning of the second night, they received one dragée placebo (n=18) or verum (n=18) in a randomized, double blind manner one hour before the lights were turned off. The confirmatory evaluation was performed by aid of the frequency analysis of the EEG recordings according to a spectral frequency index (SFx). The SFx, which is based on theta and beta powers, has been approved as an objective and reliable indicator of depth of sleep. Using this parameter, sleep was subdivided into successive periods of depth of sleep, i.e. depth of sleep below or above a SFx of 77 % (SFx above 80 % implies one is awake).

Results: The group treated with valerian in comparison to placebo generally showed longer sleeping time spent in the SFx between 77 % and 62 % and sometimes below 62 %. This was an indication of normal sleep. The probabilities of error below 77 % and below 74 % were p<0.07 and p<0.08, respectively (tested one sided). According to classical polysomnographic analysis (tested two sided), the valerian group showed less motion (p<0.05) and less sleep stage transitions (p<0.02). Tolerability of the preparation was very good.

Conclusion: Using the new parameter of „depth of sleep time” based on the spectral frequency index (SFx), it was possible for the first time to quantitatively and objectively measure the positive effects of a single dose administration of an extract of valerian root (Nervenruh Baldrian Forte) on ”bad sleepers”.
