Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) im Erwachsenenalter

Die ADHS bei Kindern persistiert bei bis zu 60 % der Betroffenen bis ins Erwachsenenalter. Sie ist für einen Teil der Betroffenen mit erheblichen Einschränkungen der Lebensqualität in verschiedenen Lebensbereichen verbunden. Insbesondere affektive Erkrankungen, Angsterkrankungen und Abhängigkeitserkrankungen gehören zum typischen Komorbiditätsspektrum.

Die Diagnose einer ADHS im Erwachsenenalter geht nicht automatisch mit einer Behandlungsindikation einher. Bei relevanten funktionellen Einschränkungen in mindestens einem Lebensbereich wird jedoch eine Behandlung empfohlen. Sie erfolgt nach Indikationsstellung als Kombinationsbehandlung aus Pharmako- und Psychotherapie.

Bis heute (Stand Juni 2007) ist in Deutschland kein Medikament für die Behandlung der ADHS im Erwachsenenalter offiziell zugelassen. Deswegen werden Medikamente zur Behandlung der ADHS nach den Richtlinien zum „Off-label use” verschrieben. Medikamente der 1. Wahl sind Stimulanzien, in Deutschland v. a. Methylphenidat. Bei Unwirksamkeit, Vorliegen von Kontraindikationen oder im Vordergrund stehenden komorbiden Erkrankungen sind selektive Noradrenalinwiederaufnahmehemmer, insbesondere Atomoxetin, Medikamente der 2. Wahl. Bei Unwirksamkeit kommen Antidepressiva, v. a. Venlafaxin und Reboxetin, als Medikamente der 3. Wahl zum Einsatz.

Verschiedene psychotherapeutische Interventionen wurden untersucht. Seit 2002 veröffentlichte Pilotuntersuchungen zu störungsspezifischen Psychotherapieansätzen zeigten sehr positive Effekte. Es existieren erste Hinweise, dass eine Kombinationsbehandlung aus Medikation und Psychotherapie wirksamer ist als eine alleinige Medikation.
