Funktionsdiagnostik in der Gastroenterologie

Kernaussagen

• Die „3 unverzichtbarsten Verfahren” der gastroenterologischen Funktionsdiagnostik sind die 24-h-pH-Metrie, die Ösophagusmanometrie und die H₂-Atemtests. Weitere Verfahren wie ¹³C-Atemtests oder die Elastasebestimmung im Stuhl sind als „sehr wünschenswert” zu bezeichnen.

24-h-pH-Metrie

• Die Hauptindikation der 24-h-pH-Metrie ist der V. a. eine gastroösophageale Refluxkrankheit bei endoskopisch negativem Befund oder bei atypischen Refluxsymptomen.

• Der prozentuale Anteil der Zeit mit einem pH < 4 und die Symptom-Reflux-Korrelation konnten als gute Parameter für die Diagnose einer gastroösophagealen Refluxkrankheit etabliert werden.

Ösophagusmanometrie

• Die Manometrie ist der Goldstandard zur Diagnosesicherung und Therapiekontrolle bei Achalasie sowie zur Abklärung einer Dysphagie bei unauffälliger Endoskopie und Verdacht auf eine Ösophagusmotilitätsstörung.

• Die Achalasie ist in der Manometrie durch eine inkomplette schluckreflektorische Relaxation des unteren Ösophagussphinkters charakterisiert.

H₂-Atemtests

• Die H₂-Konzentration in der Ausatemluft spiegelt die H₂-Produktion intestinaler Bakterien, vorwiegend im Kolon, wider. Ausgewertet wird der Anstieg der H₂-Konzentration in der endexspiratorischen Atemluft nach Substratgabe in Bezug auf einen Nüchternwert.

• Der H₂-Laktose-Atemtest ist die Standarddiagnostik zur Identifizierung einer Laktoseintoleranz, mittels eines H₂-Fruktose-Atemtests kann die hereditäre Fruktoseintoleranz diagnostiziert werden und der H₂-Laktulose-Atemtest dient der Bestimmung der orozökalen Transitzeit.
