Behandlung der chronischen Synovialfistel nach Radiosynoviorthese - Zwei Fallberichte

Zusammenfassung

Die chronische Synovialfistel am Knie ist ein insgesamt seltenes Krankheitsbild, die als Komplikation nach Trauma oder chirurgischen Eingriffen auftreten kann. Bei Patienten mit rheumatoider Arthritis ist die Ausbildung einer chronischen Synovialfistel eine bekannte und gefürchtete posttraumatische Komplikation. Eine klare und standardisierte Therapieempfehlung dieser Erkrankung steht derzeit noch aus und die bisherige Literatur auf diesem Gebiet ist nicht ausreichend. Wir berichten erstmals über zwei Patienten mit chronischer Synovialfistel nach Radiosynoviorthese des Kniegelenkes. In beiden Fällen konnte diese Komplikation unter konservativen und konventionell chirurgischen Maßnahmen nicht zur Abheilung gebracht werden, und es kam zur Ausbildung chronisch rezidivierender Fisteln. An unserer Klinik erfolgte radikales Débridement des Lokalbefundes, jedoch ohne Eröffnen des Gelenkes und bei beiden Patienten konnte mittels fasziokutaner beziehungsweise muskulokutaner Lappenplastik zur Defektdeckung ein vollständiger Wundverschluss erreicht werden.

Abstract

Chronic recurrent synovial fistulae of the knee are uncommon. They can occur after traumatic injuries, as complication after surgical treatment or in patients with rheumatoid arthritis as spontaneous complication after trauma. No consensus exists about the adequate treatment and literature on this subject is scarce. Two cases are presented in which synovial fistula had occurred after radiosynoviorthesis. Previous conservative and conventional surgery failed to solve the problem as the fistulae remained chronic and recurrent. However both, radical debridement and plastic reconstruction by either a fasciocutaneous or a myocutaneous gastrocnemius flap were successful.
