Zusammenfassung

Hintergrund: Ziel dieser prospektiven Studie war es, die möglichen Unterschiede in der Netzhautdicke der Patienten, die wegen einer chronischen Niereninsuffizienz hämodialysiert (HD) werden, und der Kontrollgruppe zu vergleichen und die Korrelation mit dem Alter und mit der Dauer der Dialysebehandlung zu analysieren. Patienten und Methoden: Unsere Studie umfasste 24 Augen von 12 HD-Patienten und 32 Augen von 16 gesunden Probanden. Die optische Kohärenztomografie (OCT) mit einem Durchmesser von 6000 μm wurde im zentralen Bereich durchgeführt. Ergebnisse: Es wurden die Mittelwerte der Netzhautdicke in den inneren temporalen, superioren, nasalen und inferioren Quadranten gemessen. Beim Vergleich beider Gruppen wurde ein höchst signifikanter Unterschied der Messungen in den inneren Quadranten gefunden (p < 0,005). Die durchschnittliche Reduktion in allen Quadranten betrug 7,7 % oder 20,1 μm (262 in der Kontrollgruppe, 241,9 bei den HD-Patienten). Die Differenz der beiden Gruppen war in allen Quadranten höchst signifikant (p < 0,001). Beim Vergleich beider Gruppen wurde kein Unterschied der Netzhautdicke im Foveabereich festgestellt. Die Reduktion der Netzhautdicke war bedeutend größer bei älteren HD-Patienten, es wurde jedoch keine Korrelation mit der Dauer der Dialysebehandlung gefunden. Schlussfolgerung: Die OCT zeigte eine signifikante Reduktion der Netzhautdicke bei den HD-Patienten. Die Reduktion war signifikant in allen Quadranten und korrelierte mit dem Alter der HD-Patienten. Im Foveabereich wurde kein Unterschied der Netzhautdicke festgestellt.

Abstract

Background: The aim of the present prospective study was to compare retinal thickness between normal healthy subjects and chronic renal failure patients treated with maintenance hemodialysis (HD) as well as to determine whether there was any correlation between age and duration of HD treatment. Patients and Methods: A total of 24 eyes of 12 HD patients and 32 eyes of 16 controls underwent optical coherence tomographic scanning (OCT) in the central disc of 6000 μm in diameter. Results: The mean retinal thickness was measured in the inner temporal, superior, nasal and inferior quadrants. There was a highly significant difference in the inner quadrants between controls and HD patients (p < 0.005). The mean retinal thickness was also measured in all outer quadrants. The differences between HD patients and controls were highly significant (p < 0.005).
