Die proximale Adduktor longus Sehnenruptur bei Leistungssportlern. Drei Fallberichte

Zusammenfassung

Chronische Leistenbeschwerden beim Sportler sind häufig die Folge von Ursprungstendinosen der Adduktoren. Abhängig von der dadurch induzierten Einschränkung der sportlichen Belastbarkeit und bei Nichtansprechen auf konservative Therapie kann eine Tenotomie indiziert werden. Im Gegensatz dazu sind akute Abrissverletzungen der Ursprungssehne des Adduktor longus jedoch selten. Bei den bisher publizierten 5 Fällen wurde immer eine Reinsertion der abgerissenen Sehne früh durchgeführt. Wir haben diese Sportverletzung, über die hier erstmalig in der deutschsprachigen Literatur berichtet wird, bei einem Hürdensprinter, einem Fußballer und einem Rock ’n’ Roll Tänzer jeweils klinisch diagnostiziert und sonografisch und kernspintomografisch bestätigt. Intraoperativ fand sich in allen Fällen ein kompletter symphysealer Abriss der Adduktor longus Sehne. Die Adduktor longus Sehne wurde mittels Knochenanker transossär anatomisch reinseriert. Nach frühfunktioneller Nachbehandlung konnte die volle sportliche Belastbarkeit nach fünf, sechs und sieben Monaten wieder erreicht werden.

Abstract

Chronic groin pain in athletes frequently results from adductor insertional tendinopathy. Adductor tenotomy has been proposed for cases recalcitrant to conservative treatment. However, acute tears of the proximal adductor tendons have rarely been described and all of them underwent early repair. This report reviews the relevant literature and adds three more acute proximal adductor longus insertional tears. Diagnosis has been made clinically and was confirmed by ultrasound and MRI. Following early repair using suture anchors and functional postoperative rehabilitation, full sports ability was re-established five, six and seven months, respectively.
