Rückfallprävention bei Psychosen: Die Umsetzung eines Selbstmanagementmodells in der ambulanten Versorgung

Dieser Beitrag stellt die Durchführung eines Projektes zur wissensbasierten, strukturierten Rückfallprävention in die Alltagspraxis des Community Mental Health Teams in Reading, Berkshire, Großbritannien vor. Er beschreibt im Detail die Pilotphase des Projekts, die kürzlich abgeschlossen wurde. Kern des Projektes ist die Verwendung von Smiths „Selbstmanagement Trainingmanual für Personen mit Psychose” zur Arbeit von Fachleuten mit Patienten, die an einer schizophrenen oder bipolaren Störung mit Psychosen leiden. Die Auswirkungen des veränderten Vorgehens werden aus der Sicht der Klienten, der Angehörigen, der Fachkräfte und der Leistungsanbieter betrachtet. Der Bericht untersucht den Veränderungsprozess sowie die Folgen für die Mitglieder des multidisziplinären Teams. Enthalten ist ein Audit der Care Programme Approach (CPA) Dokumentationsbogen sowie eine Mitarbeiterbefragung, aus denen sich Hintergrundinformationen für notwendige Verbesserung und die Mitarbeiterfortbildung ergeben. Eine Pilotstudie zur Integration des strukturierten Rückfallpräventionsprogramms in die Arbeit des gemeindepsychiatrischen Teams ist ebenfalls im Gange. Er betrachtet außerdem die Methode der Evaluation und Verbreitung im gesamten Team und in anderen Einrichtungen und er gibt Praxisempfehlungen dafür. Ziel dieses Beitrages ist es, den Hintergrund und die Entwicklung dieses laufenden Projektes vorzustellen, mit dem Schwerpunkt auf der aktuellen Pilotphase. Ergebnisse des Gesamtprojektes werden später veröffentlicht.
