Einleitung

Etwa 30 % aller Paare in Deutschland warten länger als ein Jahr auf die Erfüllung ihres Kinderwunsches, ca. 6-10 % bleiben dauerhaft ungewollt kinderlos. Seitdem Edwards und Steptoe 1978 über das erste in vitro gezeugte Kind der Welt berichteten, haben sich die Möglichkeiten, ungewollt kinderlosen Paaren zu helfen, rasant weiter entwickelt. Die Chancen auf eine erfolgreiche Behandlung haben sich über die Jahre hinweg kontinuierlich erhöht, die Risiken - bei einem vernünftigen Management - zunehmend erniedrigt. Diese Übersicht will Folgendes darstellen:

die bereits etablierten Methoden der „künstlichen Befruchtung”, neuere Entwicklungen auf dem Gebiet der Follikelreifung in vitro (In-vitro-Maturation, IVM) sowie die Fortschritte der Präimplantationsdiagnostik, v. a. die in Deutschland zugelassene Polkörperdiagnostik.

Wichtige Parameter zur Einschätzung der Fertilitätseinschränkung sind zunächst aufseiten der Frau das Alter, die Zyklusanamnese (normaler Zyklus [Abb. 1]), die Dauer des unerfüllten Kinderwunsches und aufseiten des Partners vor allem das Spermiogramm. Abhängig von der individuellen Situation können noch weitere Parameter von Bedeutung sein, so z. B. Informationen über den Genitalsitus der Frau. Generell ist zu empfehlen, die Diagnostik möglichst breit anzulegen, um alle Faktoren und Kofaktoren zu erfassen. Dementsprechend sollte ein hiervon abgeleitetes Therapiekonzept heutzutage nicht nur auf ART-Maßnahmen (assistierte Reproduktionstechniken, wie IUI, IVF oder ICSI) zielen, sondern auch die psychosozialen Implikationen der Sterilität berücksichtigen, was z. B. auch die Lebensgewohnheiten des Paares betreffen kann.

Je nach Ursache der Subfertilität bzw. Sterilität stehen folgende Therapiemethoden bzw. -techniken der assistierten Reproduktion zur Verfügung:

die intrauterine Insemination im Spontanzyklus, die intrauterine Insemination im hormonell stimulierten Zyklus, die In-vitro-Fertilisation (IVF) sowie die intrazytoplasmatische Spermatozoeninjektion (ICSI) mit Spermien aus dem Ejakulat oder Spermatozoen aus dem Nebenhoden oder Hoden, die zuvor z. B. durch eine mikrochirurgische epididymale Spermienaspiration (MESA) oder eine testikuläre Spermienextraktion (TESE), also eine Hodenbiopsie, gewonnen wurden.
