Thrombotischer Verschluß der Aorta abdominalis im Neugeborenenalter - erfolgreiche Lysebehandlung

Abstract

Aortic thrombosis during neonatal period is rare and usually related to an umbilical artery catheter. In an infant (birth weight 3110 gm) with complete thrombosis of the abdominal aorta distal to the renal arteries of unknown etiology we started a fibrinolytic therapy with streptokinase at the age of 12 days (streptokinase intravenously with an initial loading dose of 3000 U/kg, maintenance 1000 U/kg/hr for 2 days). This was followed by a heparinisation (10 000 U/ m2/d, 24 hours later 200 U/kg/d). 72 hours after onset of therapy there was no blood pressure difference between upper and lower limbs. Side-effects of therapy did not occur; the success of medical treatment continued after termination of the heparinisation

Zusammenfassung

Aortale Thrombosen sind im Neugeborenenalter selten und treten zumeist im Zusammenhang mit einem Nabelarterienkatheter auf. Bei einem Neugeborenen (Geburtsgewicht 3110 g) mit vollständigem thrombotischem Verschluß der Aorta abdominalis unterhalb der Nierenarterien aus unklarer Ursache führten wir im Alter von 12 Tagen eine fibrinolytische Therapie mit Streptokinase durch (initial 3000 E/kg KG Streptokinase i.v. in 30 min, anschließend Dauerinfusion mit 1000 E/kg/Std für 48 Std). Es schloß sich eine Heparinisierung an (10 000 E/m2/d, nach 24 Std 200 E/kg/d). 72 Std nach Therapiebeginn bestand keine Blutdruckdifferenz mehr zwischen oberen und unteren Extremitäten. Nebenwirkungen der Behandlung traten nicht auf, und der Therapieerfolg blieb nach Beendigung der Heparinisierung erhalten
