Abstract

Nasopharyngeal carcinomas (NPCs) are malignant tumors which exhibit a wide disparity in their age, racial, and geographic incidence. In parts of Africa NPCs account for 10% to 20% of childhood malignancies. In USA and Europe, the NCP is an uncommon tumor (0.2% of all malignancies) and amounts to only 1% to 2% of childhood malignancies. Etiology and pathogenesis are closely related to an infection with Epstein-Barr Virus (EBV) and the EBV genome was detected in tumor tissues. Children with NPC differ from their adult counterparts in having a closer association with Epstein-Barr-Virus-Infections. The classical lymphoepithelial carcinomas (Cologne type II-type III) have been found in young patients. Clinically, the disease is aggressive, characterised by frequent metastases in bone and lung. These carcinomas are associated with significantly elevated anti-EBV-titers. The prognosis of children with advanced NPC is poor with a 5-year survival rate between 20-30%. Radiotherapy is the treatment of choice in NPC which has provided an improvement in local tumor control in recent years. Human fibroblast interferon is an active agent in recurrent NPC. Seven children have been treated with IFN-β, (6 with human und 1 with recombinant IFN-β) as an adjuvant therapy in dosis of 105 U/kg body weight three times a week for half a year. All patients received radiotherapy to primary site and had advanced stages (III-IV) at presentation. The patients' age ranged from 14-19 years at diagnosis. Six are still in CR (RFS are 10, 8, 8, 7, 6 and 1,5 years) and one patient relapsed after 18 months. The efficacy of preradiation chemotherapy and of adjuvant IFN-therapy in advanced stage of nasopharyngeal carcinoma is proved and should improve the relaps free survival, especially in the high risk groups. The preliminary results including tolerable side effects of IFN and preradiation chemotherapy show, that a clinical trial is urgently needed and a prospectiv, controlled trial will be presented.

Zusammenfassung

Die Nasopharynxkarzinome sind maligne Tumoren, die eine große Streubreite im Alter und der geographischen Inzidenz aufweisen. In Teilen Afrikas ist es mit 10-20% ein häufiger Tumor im Kindesalter. In USA und Europa jedoch ist das Nasopharynxkarzinom ein seltener Tumor im Kindesalter. Die Inzidenz beträgt 0,2% aller Tumoren. Die Ätiologie und Pathogenese ist eng assoziiert mit einer EBV-Infektion, und das EBV Genom läßt sich im Tumorgewebe nachweisen. Histologisch findet man bei Kindern und Jugendlichen das klassische lymphoepitheale Karzinom (WHO-Klassifikation Typ II-III). Nasopharynxkarzinome weisen ein sehr aggressives Wachstum auf und metastasieren häufig in Knochen und Lunge. Die Prognose der Kinder im fortgeschrittenen Tumorstadium ist schlecht, die 5-Jahre Überlebensrate beträgt 20-30%. Die Bestrahlung ist die Therapie der Wahl und zeigt eine gute Tumorlokalkontrolle. Die Behandlung rezidivierter Nasopharynxkarzinome mit Interferon ergab eine erfreuliche Ansprechrate. 7 Patienten wurden adjuvant mit humanem, bzw. rekombinantem Interferon in einer Dosis von 105 IE/kg/KG 3× wöchentlich über ein halbes Jahr behandelt. Alle Patienten erhielten eine Bestrahlung auf den Primärtumor und befanden sich zum Zeitpunkt der Diagnose im Stadium III oder IV. Das Patientenalter betrug 14-19 Jahre, 6 befinden sich in anhaltender kompletter Remission (RFS: 10, 8, 8, 7, 6, 1,5). 1 Patient rezidivierte bereits nach 18 Monaten. In vielen kleineren Studien konnte die Wirksamkeit einer Cisplatin gestützten Chemotherapie nachgewiesen werden. Die Effektivität sowohl einer Chemotherapie vor Bestrahlung, als auch einer adjuvanten Interferon-Therapie sollte bei Patienten mit Nasopharynxkarzinomen im fortgeschrittenen Stadium überprüft werden, um die Überlebensrate, besonders bei den ,,high risk-Patienten" zu verbessern. Die vorläufigen Ergebnisse und die geringe Nebenwirkungsrate des Interferons und der Chemotherapie vor Bestrahlung zeigen, daß eine klinische Studie notwendig ist. Eine prospektive kontrollierte Studie wird vorgestellt.
