Zusammenfassung
Fragestellung: Bei Patientinnen mit Präeklampsie und HELLP-Syndrom wurden erhöhte Serumspiegel von tissue-Plasminogen Activator (t-PA) und Plasminogen Activator lnhibitor-1 (PAI-1) nachgewiesen. Ziel der vorliegenden Studie war es, die Frage zu klären, ob diese Ergebnisse Ausdruck einer Endotheischädigung oder einer Endothelaktivierung mit gesteigerter Produktion von t-PA und PAI-1 sind. Material und Methodik: Humane Endothelzellen wurden aus Nabelvenen (HUVEC) kultiviert und für 24 h mit Serum von Patientinnen mit HELLP-Syndrom (n = 7) bzw. Präeklampsie (n = 12) stimuliert. Als Kontrollen dienten die Seren von 12 normotensiven Schwangeren und 9 Nichtschwangeren. Daneben wurden unstimulierte Endothelzellen mittels Aqua dest. zur Lyse gebracht. Die Bestimmung von t-PA und PAI-1 erfolgte mittels ELISA sowohl im mütterlichen Serum als auch im Überstand sowie im Lysat der Endothelzellkulturen. Ergebnisse: Im Überstand der Endothelzellkulturen traten nach Stimulation mit Gestoseserum im Median signifikant höhere Werte, sowohl für t-PA (HELLP: 71,2 ng/mg Protein bzw. Präeklampsie: 61,0 ng/mg Protein versus normotensive Schwangere: 40,6 ng/mg Protein bzw. Nichtschwangere: 37,0 ng/mg Protein [p <0,001]) als auch für PAI-1 (HELLP: 13023 ng/mg Protein bzw. Präeklampsie: 12691 ng/mg Protein versus normotensive Schwangere: 7634 ng/mg Protein bzw. Nichtschwangere: 9005 ng/mg Protein [p < 0,01 ]) auf. Im Lysat der Endothelzellen konnte PAI-1 lediglich in einer Konzentration von 219 ng/mg Protein gemessen werden, während t-PA nicht nachweisbar war. Schlußfolgerung: Da nach Lyse der unstimulierten Endothelzellen kein t-PA und nur geringe Mengen an PAI-1 gemessen werden konnte, deutet die gesteigerte Expression von t-PA und PAI-1 aus humanen Endothelzellen nach Stimulation mit Serum von Gestosepatientinnen auf eine gesteigerte Endothelzellaktivierung hin.
Abstract
The aim of our study was to determine the potential cytotoxicity of sera from patients with preeclampsia and HELLP Syndrome, by investigating the influence of these sera on the expression of tissue-Plasminogen Activator (t-PA) and Plasminogen Activator lnhibitor-1 (PAI-1) into the supernatant of cultured human endothelial cells. For this purpose, we stimulated human endothelial cells, obtained from umbilical veins (HUVEC), over a period of 24 hours with sera of seven patients with HELLP-Syndrome and 12 patients with preeclampsia. The sera of 12 normotensive pregnant patients and nine non-pregnant women served as controls. We determined t-PA and PAI-1 levels by ELISA, both in the maternal serum and in the supernatant of HUVEC. While t-PA levels were significantly higher in the serum of patients with preeclampsia and HELLP Syndrome when com-pared to the controls (median: 8.0 ng/ml and 6.2 ng/ml vs. 3.9 ng/ml and 3.1 ng/ml), levels of PAI-1 did not show any significant differences between the study groups. The supernatant of the endothelial cell cultures demonstrated significantly elevated levels of both t-PA (p < 0.001) and PIA-1 (p <0.01) when stimulated with serum obtained from patients with preeclampsia or HELLP-syndrome. The increased expression of t-PA and PAI-1 from human endothelium cells after Stimulation with serum of preeclamptic patients supplies evidence of increased endothelial cell activation.
