Ovulation bei Hypergonadotropinämie: eine kasuistische Betrachtung

Zusammenfassung
Wir berichten über eine Verlaufsbeobachtung von FSH. LH und Estradiol (E2) bei einer Patientin mit Resistant-Ovary-Syndrom. Die Ausgangswerte von FSH lagen zwischen 28 und 80 IE/l, nach Gaben von Leuprorelin (Lpr) und Humanpausengonadotropin (HMC) zwischen 5 und 12 IE/l. Die Basalwerte von LH schwankten zwischen 30 und 70 IE/l. Nach der Therapie lagen sie bei 0.7 und 9 IE/l. E2 stieg auf Injektionen von HMG an, doch fiel die Reaktion zunehmend geringer aus. Nach einem Jahr ovulierte die Patientin nicht nur auf HMG. sondern auch spontan. Schlußfolgerung: Das Resistant-Ovary-Syndrom ist eine Variante des Climacterium präcox, bei der der Follikelapparat des Ovars erhalten ist. und spontane Remissionen möglich sind.
Abstract
A Case Report: Considerable changes of serum FSH, LH and E2 in response to hormonal treatment were recorded in a patient with resistantovary syndrome during one year of observation. The FSH levels ranged initially from 28 to 180 IU/l: after treatment leuproreline (Lpr) and HMG. respectively. they decreased to 5 to 12 IU/l. Serum E2 reacted to injections of HMG with a marked increase. but the amplitude of the response diminished in subsequent treatment cycles. After one year of observation, the patient ovulated spontaneously several times. Conclusion: The resistant-ovary syndrome is a variant of premature menopause characterised by a normal supplement of unresponsive follicles which may, however, termporarily regain normal function.
