Zusammenfassung
Bei den Multi-System-Degenerationen werden neben den klinischen Zeichen eines Parkinson-Syndroms weitere neurologische Symptome gefunden, die einen über das extrapyramidale System hinausgehenden Prozeß anzeigen.
Zu diesen sogenannten Parkinson-Plus-Syndromen gehört das klinisch durch eine orthostatische Kollapsneigung gekennzeichnete Shy-Drager-Syndrom. Ein Fall mit typischen klinischem Bild des Shy-Drager-Syndroms wird vorgestellt. Zur Diagnose des Shy-Drager-Syndroms ist die Prüfung der kardiovaskulären Anpassungsreaktion bei orthostatischer Belastung wichtig. Der basale Noradrenalin-Plasmaspiegel ist beim Shy-Drager-Syndrom nicht erniedrigt, läßt sich aber durch Aufrichten nicht stimulieren.
Die orthostatische Hypotonie kann medikamentös nur vorübergehend gebessert werden. Gegen die extrapyramidalen Symptome im Rahmen des Shy-Drager-Syndroms sollten anticholinerge Substanzen und Amantadin versucht werden. Wegen des hypotonie-verstärkenden Effektes kommt es unter L-Dopa häufig zu einer Verschlechterung des klinischen Bildes.

Parkinsonism-plus syndromes are characterized by neurological symptoms indicating a degenerative disease of the central nervous system that is an extension of Parkinson's disease affected substantia nigra. The Shy-Drager syndrome belongs to the Parkinsonism-plus syndromes. Orthostatic hypotension, sphincteric disturbances, other dysautonomic features, and a movement disorder, usually parkinsonism, constitute the Shy-Drager syndrome. A case with typical clinical features and course is presented.
Testing of autonomic functions, especially the cardiovascular reaction on tilting, is important to diagnose Shy-Drager syndrome.
As degeneration of preganglionic autonomic neurons resting plasma norepinephrine levels are normal in Shy-Drager syndrome, but there is no significant rise of plasma noradrenaline on tilting.
Drugs and physical therapy can only temporary improve the orthostatic hypotension and supine hypertension limits their usefulness in this progressive disease. Dopaminergic drugs exacerbate orthostatic hypotension and should not be used to improve extrapyramidal features in Shy-Drager syndrome. Contrary to this anticholinergic drugs and amantadine have no affect on blood pressure.
