Summary
The time course of body temperature (T), white blood cell count (WBC) and serum C-reactive protein (CRP) following uncomplicated open-heart surgery and the value of these three parameters in the diagnosis of postoperative infectious complications were analyzed in 153 patients. Preoperatively, all the patients had a normal T and the WBC level was under 10 × 109 /l. The CRP was elevated (over 20 mg/1) in five patients without having an influence on their postoperative courses. Postoperatively, T, WBC and CRP reached their maximum values during the first three days, decreasing gradually thereafter. Great individual variations in this average response to open-heart surgery were seen. Development of minor postoperative infections, mainly wound infection, (in 22 patients) were poorly reflected by these three parameters. However, a temperature exceeding 38°C after the 6th postoperative day suggested presence of infection; less than 5% of patients without an infection had a temperature over this value. A secondary rise in CRP after the third postoperative day occurred in 20 patients and correlated well with postoperative complications; in 14 patients (70%) an infection became apparent, in three instances a probable infection was present, while two patients suffered from non-infectious complications. Only in one case (5%) did the cause for a secondary rise in CRP remain unknown. In major infections, i. e. mediastinitis, a CRP response occurred in all 12 patients, a temperature response in 11 patients (92%) and a WBC response in 10 patients (83%).
Zusammenfassung
An 153 Patienten wurde der zeitliche Verlauf von Körpertemperatur, Leukozytenzahl und C-reaktivem Protein im Serum (CRP) nach unkomplizierten offenen herzchirurgischen Eingriffen untersucht (85 koronar-chirurgische Operationen, 61 Klappenersatzoperationen und 7 andere).
Es sollte ergründet werden, welchen Wert diese Parameter in der Diagnostik postoperativer Infektionen haben.
Präoperativ hatten alle Patienten normale Temperaturen und eine Leukozytenzahl unter 10 × 109 /l. Für das CRP wurde ein Normalwert von 10 mg/l festgelegt. Fünf Patienten hatten bereits präoperativ einen CRP-Wert von über 20 mg/1, ohne daß der postoperative Verlauf auffällig gewesen wäre. Die Maximalwerte aller drei Parameter wurden während der ersten 3 postoperativen Tage erreicht, danach fielen die Werte kontinuierlich ab. Es wurden große individuelle Schwankungen beobachtet. Das Krankengut wurde in 3 Gruppen eingeteilt: die 1. Gruppe ohne postoperative Infektionen (115 Patienten), die 2. Gruppe mit geringfügigen Infektionen (22 Patienten) und die 3. Gruppe (16 Patienten) mit ernsten postoperativen Infektionen, davon 12 mit einer Mediastinitis. Bei einer ernsten postoperativen Komplikation fanden sich nach dem 6. postoperativen Tag in über 90% der Fälle Temperaturen über 38° C; in gleichem Sinn war ein sekundärer CRP-Anstieg nach dem 3. postoperativen Tag und eine Leukozytose zu werten. Der stark erhöhte CRP- Wert jenseits des 3. postoperativen Tages hat einen hohen diagnostischen Wert, ihm liegt meistens eine Mediastinitis zugrunde. Fehlt eine solche, so ist an eine noch floride Endokarditis zu denken.
