Summary
Nowadays the acute and especially chronic lung rejection are the major problems after lung transplantation (L-Tx) with relevant influence on longterm survival. We performed lung transplantation in rats to study a possible role of ultrastructural lesions in the graft during the acute rejection process, concerning their reversibility/irreversibility and influence of the chronic rejection.
Based on histologic and immunohistologic studies after L-Tx in MHC-different and strong reactive rat strain combination AVN-LEW and filial generation (AVN-LEW)F1-LEW (n = 57 and n = 32) electronmicroscopic studies (TEM, SEM) were performed in the combination AVN-LW (n = 20) on postoperative day 0, 1, 2 and 5, all without immunsuppressive therapy. Syngenic grafts (LEW-LEW; n = 12) served as controls.
Histologically the allografts were classed according to the proven acute rejection phases latent, vascular, alveolar and destructive. The immunhistological and electronmicroscopic results correlated with these rejection phases. There was no difference between the rat strain combinations. All allografts developed acute rejection on postoperative day 2 and were destroyed on postoperative day 5/6. Initially T-helper-cells, later cytotoxic-T-cells and macrophages played the predominent role in the acute rejection process. In the ultrastructural specimens alterations of the blood vessels, pneumocytes type-II, and surfactant gave more information. Initially flattening of endothelial cells and circumscribed lesions of graft vessels occur, increasing in the allografts up to extensive vascular wall destructions, accompanied by total thrombotic occlusion. Disturbances of Surfactant production observed in the grafts of all strain combinations are not homogenous. In the syngenic grafts Surfactant in the pneumocytes type-Il and in the alveoli is completely restored until postoperative day 12, whereas in the allografts complete disturbance of surfactant production remained for the whole study period. The protracted Surfactant depletion results in increasing dys- and atelectasis of the pulmonary parenchyma. Substitution of Surfactant after L-Tx seems to be an important measure.
Zusammenfassung
Die akute und insbesondere chronische Abstoßung sind heutzutage die im Vordergrund stehenden Probleme nach Lungen-transplantation (L-Tx) mit wesentlichem Einfluß auf das Langzeitüberleben. Aus diesem Grunde haben wir ultrastrukturelle Läsionen im Transplantat während der akuten Abstoßungsreaktion im Hinblick auf Reversibilität/Irreversibilität und Bedeutung für die chronische Abstoßung studiert.
Aufbauend auf histologischen/immunhistologischen Studien nach L-Tx in der MHC-differenten und stark reaktiven Ratten-Stammkombination AVN-LEW und Filialgeneration (AVN LEW)F1-LEW (n = 57 und n = 32) wurden elektronenmikroskopische Untersuchungen (TEM, REM) in der Kombination AVN-LEW (n = 20) am Tage 0,1,2 und 5 nach L-Tx ohne Einsatz einer immunsuppressiven Therapie vorgenommen. Syngene Transplantate (LEW-LEW; n = 12) dienten als Kontrolle.
Histologisch wurden die Transplantate den bewährten Abstoßungsphasen latent, vaskulär, alveolär und destruktiv zugeordnet. Die immunhistologischen und elektronenmikroskopischen Befunde korrelierten mit den Abstoßungsphasen. Es war kein Unterschied zwischen den allogenen Stammkombinationen nachweisbar. Alle Allo-Transplantate entwickelten eine akute Abstoßungsreaktion am Tage 2 und waren am Tage 5/6 nach L-Tx destruiert. Initial spielten T-Helfer-Zellen, später zytotoxische T-Zellen und Makrophagen die Hauptrolle im Abstoßungsprozeß. In der Ultrastruktur ergaben sich zusätzlich Informationen bei den Gefäßläsionen, den Pneumozyten Typ II und beim Surfactant. Initial entstehen Endothelzellabflachungen und umschriebene Endothelläsionen in den Gefäßen in allen Transplantaten, die bei den allogenen Transplantaten zu ausgedehnten Gefäßwanddestruktionen mit thrombotischen Verschlüssen führen. Die Surfactantbildungsstörungen sind nicht einheitlich nachweisbar. In den syngenen Transplantaten ist der Surfactant in den Pneumozyten Typ II und intraalveolär bis zum 12. Tag nach L-Tx wiederhergestellt, wohingegen sich in den allogenen Transplantaten eine komplette Surfactantbildungsstörung über den gesamten Untersuchungszeitraum einstellt. Folgen sind zunehmend Dys- und Atelektasen im Transplantat. Therapeutische Option wäre dementsprechend die lokale Substitutionsbehandlung mit Surfactant nach L-Tx.
