Venöse Stentapplikation mit simultanem kubitofemoralen Zugang

Zusammenfassung

Bei 4 Patienten mit oberer Einflußstauung, die durch Stenose oder Verschluß der zentralen Venen verursacht wurde, war eine Passage des Führungsdrahtes von transfornoral nicht möglich. Daraufhin wurde ein zusätzlicher transbrachialer Zugang angelegt. In allen Fällen gelang die Passage der Venen von transbrachial. Nach Durchzug des überlangen Führungsdrahtes durch die noch liegende transfemorale Schleuse schloß sich bei allen 4 Patienten eine erfolgreiche Wallstent-Applikation von transfemoral aus an. Peri- oder postinterventionelle Komplikationen wurden im Überwachungszeitraum von 2 Tagen nicht beobachtet.

Summary

In 4 patients with superior inflow obstruction caused by stenosis or occlusion of the central veins it was not possible to pass the guide wire from transfemoral. An additional transbrachial access was applied to avoid the stenosis or occlusions. Patency of the veins from transbrachial was achieved in all cases. After pulling the extensively long guide wire through the still horizontal venous lock, successful Wallstent application was performed from transfemoral in all 4 patients. No periinterventional or post-interventional complications were seen during 2 days follow-up.
