Zusammenfassung
Es wird die Problematik der Auswahl für die klinische Praxis aus dem breiten Spektrum der angebotenen Antibiotika und der notwendigerweise oft ungezielten Initialtherapie besprochen. In einer Übersicht wird auf die für die Therapie im Krankenhaus wichtigen Antibiotikagruppen eingegangen: Penicilline, Aminoglykoside, Metronidazol und Cephalosporine. Zunächst wird das Wirkungsspektrum der jeweiligen Antibiotikagruppe vereinfachend zusammengefaßt, woraus sich dann die Indikationen ergeben. Bei der Besprechung des klinischen Einsatzes der Antibiotika wird das Schwergewicht auf die Initialtherapie vor oder ohne Erregerkenntnis bei lebensbedrohlichen Infektionserkrankungen gelegt. Unter diesem Gesichtspunkt wird die Therapie der Pneumonie, der Peritonitis, der Sepsis und Urosepsis und der Meningitis besprochen. Kurz wird auf die wenigen Indikationen zur Antibiotikaprophylaxe eingegangen. Abschließend wird eine Auswahl von Antibiotika vorgestellt, die für die Therapie im Krankenhaus notwendig sind.
Summary
The problems of selecting antibiotics for hospital use from the broad variety offered, and the problems of what is often necessarily a non-specific initial therapy, are discussed. A review is given of groups of antibiotics which are important for hospital therapy such as penicillins, aminoglycosides, metronidazol and cephalosporins. Firstly the range of effects of the individual groups of antibiotics is summarized in a simplified form from which the indications are then derived. In discussing the clinical use of antibiotics special importance is attached to initial therapy prior to or in the absence of identification of the pathogens in fatal infectious diseases. From this aspect the therapy for pneumonia, peritonitis, sepsis and urosepsis as well as meningitis is discussed. The small number of indications for the prophylactic use of antibiotics is briefly dealt with. Finally a selection of antibiotics necessary for hospital therapy is presented.
