Abstract

Ischemic ophthalmopathy is the leading ocular symptom of occlusive processes of the internal carotid artery. It can be preceded by embolic symptoms as amaurosis fugax or occlusion of a branch or the main stem of the central retinal artery. Both, ophthalmologists and neurologists, should be familiar with these disturbances as they can involve important diagnostic and therapeutic consequences.

Five out of six patients with an angiographically demonstrated extracranial occlusion of one internal carotid artery and a retrograde collateral circulation to the intracranial space via the external carotid and the ophthalmic arteries showed an ischemic ophthalmopathy of the ipsilateral eye. This high coincidence supports the notion that an ischemic ophthalmopathy may not be the result of the occlusive process of the internal carotid artery per se, but may be caused by ocular ,,steal mechanisms" due to the retrograde orbital collateral circulation.

Zusammenfassung

Ophthalmologisches Leitsymptom stenosierender Prozesse der A. carotis interna ist die ischämische Ophthalmopathie. Embolisch bedingte Symptome wie Amaurosis fugax oder der Verschluß eines Astes oder des Hauptstammes der A. centralis retinae können vorangehen. Die Kenntnis dieser Störungen ist für den Ophthalmologen ebenso wichtig wie für den Neurologen, da sich daraus entscheidende diagnostische und therapeutische Konsequenzen ergeben können.

Bei 5 von 6 Patienten, bei denen wir angiographisch einen Verschluß der A. carotis interna extrakraniell und einen retrograden Kollateralkreislauf über die A. carotis externa A. ophthalmica nach intrakraniell nachweisen konnten, stellten wir eine ischämische Ophthalmopathie am ipsilateralen Auge fest. Diese hohe Koinzidenz stützt die Annahme, daß die ischämische Ophthalmopathie nicht durch den stenosierenden Prozeß in der A. carotis interna per se, sondern erst infolge von okulären ,,Steal-Mechanisme" bei retrogradem orbitalen Kollateralkreislauf entsteht.
