Zusammenfassung
38 Kinder im Alter von 2 - 13 Jahren (Körpergewicht 10 - 28 kg) wurden mit intranasal appliziertem Midazolam in einer Dosis von 0,2 mg/kg vor HNO-ärztlichen Eingriffen in Allgemeinanästhesie prämediziert. Intranasal appliziertes Midazolam zeigte eine rasch einsetzende Wirkung: 79 % bzw. 95 % der Kinder waren nach 5 Minuten (p < 0,05) bzw. nach 10 Minuten (p < 0,01) ausreichend prämediziert gegenüber 45 % vor der Prämedikation. Der Befund steht im Einklang mit pharmakokinetischen Daten: Intranasal appliziertes Midazolam erreicht maximale Wirkspiegel nach 11 Minuten, die Bioverfügbarkeit liegt bei 51 % (10). Durch die Entwicklung einer konzentrierten Lösung von Midazolam wäre die intranasale Applikation auch für die Prämedikation Erwachsener nutzbar.
Summary
The effect of 0.2 mg/kg body weight intranasal midazolam as a premedicant was investigated in 38 children (age 2 - 13 years, body weight 10 - 28 kg) prior to ENT surgery. Intranasal midazolam showed a rapid onset of action with 79 % and 95 % of the children being sufficiently premedicated at 5 minutes (p < 0.05) and 10 minutes (p < 0.01) after application, respectively, as opposed to 45 % prior to this. This finding is in keeping with pharmacokinetics showing a tmax of intranasal midazolam of 11 minutes and a biovailability of 51 % in children (10). With the advent of a higher concentrated solution of midazolam intranasal application is also concevable as a premedication in adults.
