Autoimmunerkrankungen der Leber

Da einige klinische und histologische Charakteristika der AIH auch bei der PSC und der PBC zu finden sind, wurden diagnostische Kriterien für die AIH festgelegt. Der AIH-Typ 1 weist ANA und/oder SMA auf, der AIH-Typ 2 LKM-1.

Für die PSC gibt es keinen spezifischen Marker, Goldstandard der Diagnostik ist die ERCP.

Die PBC tritt sowohl beim Sjögren-Syndrom als auch bei der Hashimoto-Thyreoiditis, dem Morbus Basedow und der Sprue gehäuft auf. Klinisch steht der Pruritus im Vordergrund.

Endokrinologische Erkrankungen

Der Morbus Basedow ist häufig mit anderen Autoimmunerkrankungen wie Vitiligo, Myasthenia gravis, Morbus Addison, atrophischer Gastritis, Typ-1-Diabetes, Sprue und PBC assoziiert.

Autoimmunthyreoiditiden sind oft mit einer primären Autoimmunerkrankung der Leber assoziiert, v. a. mit der PBC und der AIH. Eine aktive Autoimmunhepatitis kann eine milde Hypothyreose maskieren.

Aufgrund des gehäuften Auftretens weiterer Autoimmunerkrankungen bei Diabetes mellitus Typ 1 sollte bei erhöhten Leberwerten auch immer der Verdacht auf eine Autoimmunhepatitis bestehen.

Darmerkrankungen

CED sind am häufigsten mit einer PSC assoziiert, es kommen aber auch chronische Hepatitiden, Fettleber und PBC vor.

Im Rahmen einer Sprue kann sich eine chronische Hepatitis entwickeln, und es konnte das gehäufte Auftreten einer PBC beobachtet werden.

Kollagenosen

Aufgrund des gleichzeitigen Auftretens von ANA, SMA und anti-dsDNS-AK bei Patienten mit AIH und bei Patienten mit SLE lässt sich häufig nicht differenzieren, ob eine AIH mit systemischer Beteiligung oder ein SLE mit Leberbeteiligung vorliegt.

Das Sjögren-Syndrom tritt sekundär als „Sicca-Syndrom” häufig bei anderen Kollagenosen und rheumatoider Arthritis, aber auch bei einer Hepatitis-C-Virusinfektion und bei PBC auf.
