  • Unter infektionspräventiven Aspekten wird bei den nosokomialen Infektionen die Übertragung durch Kontakt, Tröpfchen und Tröpfchenkerne (Aerosole) unterschieden.

  • Als effektive Maßnahme zur Prävention der Übertragung nosokomialer Infektionen hat sich zunächst die Etablierung so genannter „Standardmaßnahmen” bzw. der „Standardhygiene” bewährt, die von allen Angehörigen des medizinischen Personal bei jedem Patienten anzuwenden sind.

  • Wichtigster Bestandteil der Standardhygiene ist die Händedesinfektion, die als kosteneffektivste Maßnahme zur Prävention der Übertragung nosokomialer Infektionen gilt.

  • Bei bestimmten Infektionskrankheiten oder -erregern sollten zusätzlich zur Standardhygiene bestimmte „übertragungsspezifische Maßnahmen” durchgeführt werden, die entsprechend dem jeweiligen Übertragungsmodus als „Kontaktisolierung”, „Tröpfchenisolierung” oder „aerogene Isolierung” bezeichnet werden.

  • Besonders die Unterscheidung zwischen der Tröpfchenisolierung und der aerogenen Isolierung ist trotz ihrer nachweislichen infektiologischen Relevanz in Deutschland weitgehend unbekannt.

  • Die wichtigste Maßnahme zur Prävention der nosokomialen Übertragung multiresistenter Erreger ist die konsequente Umsetzung der Standardhygiene, insbesondere der Händedesinfektion. Die Bedeutung der räumlichen Isolierung ist unklar; das Tragen von OP-Masken und Schutzkleidung ist gegenüber der Händedesinfektion von untergeordneter Bedeutung.
