Zusammenfassung
Hintergrund Die Psychose gehört zu den schwerwiegendsten mit einer Epilepsie assoziierten psychischen Veränderungen. Unter den verschiedenen Varianten kommen postiktale Psychosen am häufigsten vor. Methode Wir berichten über einen Patienten mit einer paranoid-halluzinatorischen Psychose, die mit einem Intervall nach einem prolongierten komplex-fokalen Anfall auftrat und nach 3 Tagen vollständig regredient war. Anhand der Falldarstellung werden Klinik, apparative Diagnostik, Differenzialdiagnosen und Therapie diskutiert. Ergebnisse Das EEG ist bei der Diagnostik obligatorisch. Ziel ist die Optimierung der antiepileptischen Medikation zur Anfallsreduktion, um damit die postiktale Psychose zu verhindern. Schlussfolgerung Dieser Fall zeigt, dass eine enge Zusammenarbeit zwischen Neurologen und Psychiatern bei Epilepsie assoziierten Psychosen für einen optimalen Therapieerfolg entscheidend ist.
Abstract
Background Postictal psychosis is an important complication in patients with epilepsy. Method We report a male patient with pre-existing epilepsy who developed acute psychosis with delusions and acoustic hallucinations, two days after a prolonged complex-partial seizure. Upon treatment with diazepam and haloperidol, the psychosis subsided within three days. Result The optimation of antiepileptic drugs is the main point to prevent seizures and postictal psychotic episodes in epilepsy. Conclusion This case shows that close collaboration between neurologists and psychiatrists on epilepsy associated psychoses is decisive for an optimal success of therapy.
