Zusammenfassung

Das subakromiale Impingement beschreibt den pathologischen Kontakt der Rotatorenmanschette mit dem osteofibrösen Schulterdach. Es ist damit lediglich ein Symptom und kein Sammelbegriff für die heterogene Gruppe der Erkrankungen des subakromialen Raumes. Ziel der Diagnostik ist es, die Genese des Impingements zu verifizieren. Die Unterscheidung struktureller von funktionellen Ursachen ist essenziell. Die Therapie richtet sich nach der Genese. Die Diagnose „Impingementsyndrom“ darf nicht unreflektiert eine Akromioplastik nach sich ziehen. Da der Begriff „Impingementsyndrom“ keinen kausalen Bezug hat, sollte er als Diagnose nicht mehr verwendet werden. Der Begriff des Subakromialsyndroms mit zusätzlicher Nennung der zugrunde liegenden Pathologie verschafft mehr therapeutische und prognostische Klarheit.
