In-vivo-Vergleich verschiedener approximaler Abschrägungsverfahren bei Goldrestaurationen

Zusammenfassung

Das Ziel der vorliegenden Untersuchung war es, die Präparationsqualität unterschiedlicher approximaler Abschrägungstechniken für Goldgussinlays oder -teilkronen mit einer neu entwickelten Methode in vivo bezüglich Rand- und Oberflächenqualität der Abschrägung zu vergleichen. Es kam die konventionelle rotierende Abschrägungspräparation mit Hohlkehlflammen zum Einsatz. Als niederfrequent schwingendes Verfahren wurden Bevelshape-Feilen (B40C, Intensiv, Lugano, Schweiz) im Prepcontrol-Kopf (61 LR, KaVo, Biberach, Deutschland) und als neues hochfrequent oszillierendes Verfahren die teildiamantierten Sonic-prep-gold-Ansätze im Sonicflex-Airscaler (beides KaVo) angewendet. Von 7 erfahrenen Zahnärzten unserer Klinik wurden 60 approximale Kavitäten, d. h. 20 je Methode, abgeschrägt. Die Replika-Beurteilung der Abschrägungsränder bzw. -oberflächen erfolgte mittels Vermessungssoftware quantitativ-qualitativ im Rasterelektronenmikroskop. Die statistische Auswertung der Daten erfolgte anhand nonparametrischer Testverfahren. Mit jeder der 3 Methoden wurden bezüglich Randqualität über 90 % „perfekte Randanteile ohne Defekte” erreicht. Der Sonic-prep-gold-Ansatz wurde im Vergleich zur rotierenden Bearbeitung oder Feilenpräparation tendenziell bzw. signifikant besser bezüglich Randverlauf und Oberflächenqualität eingeschätzt. Die sonoabrasive Abschrägungspräparation für Goldrestaurationen (Sonic-prep-gold-Ansatz) zeichnet sich durch defektarme, harmonisch verlaufende Ränder und regelmäßige Oberflächen aus.

Summary

Comparison of different preparation methods for bevels of class II cast metal restorations in vivo. The purpose of this study was to develop a new method for bevel preparation for Class II cast metal inlays or onlays and to evaluate the margin and surface quality. The new partially diamond-coated preparation tip Sonic-prep-gold (KaVo, Biberach, Germany) is oscillated at a high frequency in the sonic range (6,5 kHz) and driven by a pneumatic Airscaler (Sonicflex 2000N, KaVo). In this in vivo study this sonoabrasive tip for beveling Class II box shaped cavities was compared to a rotating preparation technique with a flame-shaped, finegrit diamond instrument and another oscillating technique, based on the EVA-System (Prepcontrol 61 LR, KaVo) with the diamond-coated Bevelshape®-file (B40 C, Intensiv, Lugano, Switzerland).

With each method 20 bevel preparations were clinically done by 7 dentists. The margins and surfaces of the bevels were evaluated quantitatively using the SEM. The statistical comparison of the 3 methods was performed by non-parametric test procedures. Each of the 3 methods had over 90 % perfect margins without marginal defects. After bevel preparation with the Sonic-prep-gold-instrument the quality of the marginal contours and surfaces was significant better than after preparation technique with the Bevelshape®-file and the rotating instruments.

We conclude that the newly designed sonoabrasive tip for cast metal inlays (Sonic-prep-gold) allows the preparation of interproximal bevels with excellently finished margins, plain marginal contours and smooth surfaces.
