Purpose: To present typical imaging features of mucocele of the appendix.
Methods and Materials: A 53-year-old man was admitted to the hospital suffering from abdominal pain and swelling in the right lower quadrant. Ultrasonography (US) and multidetector computerized tomography (MDCT) examinations were performed.
Results: US examination revealed an oval shaped, 10×5cm cystic mass at the paracecal area. Also, concentric echogenic layers (“onion skin appearance“) within the cystic mass, which is characteristic for appendiceal mucocele, were detected. CT examination also confirmed this cystic lesion with tiny capsular calcifications.
Conclusions: A sonographically layered cystic mass in the right lower quadrant of the abdomen suggests the diagnosis of the appendiceal mucocele, especially in male patients. Recognition of the sonographic “onion skin sign“ in the cystic mass facilitate the accurate preoperative diagnosis.
