Perineale Urethrektomie beim Mann

Einleitung

Die häufigste und onkologisch wichtigste Indikation zur Urethrektomie ist der prostatische Harnröhrenbefall. Beim multifokalen Blasenkarzinom, Tumor im Blasenhals und Carcinoma in situ ist grundsätzlich die Möglichkeit zum Harnröhrenerhalt und orthotopen Blasenersatz bei entsprechend sorgfältiger onkologischer Nachsorge und Aufklärung des Patienten möglich. Die einzige absolute Kontraindikation für den orthotopen Harnblasenersatz ist das Vorhandensein von Karzinomanteilen in der anterioren Harnröhre und der positive Harnröhrenabsetzungsrand im Schnellschnitt während der Zystoprostatektomie [10].

Die Häufigkeit des Harnröhrenrezidives nach Zystoprostatektomie liegt auch in den neueren Arbeiten zwischen 4 % und 11 % [3] [5 7] [9] [12]. Es kann insbesondere bei der prostatischen Stromainvasion zu einer Verschlechterung der Prognose der Patienten führen.

Den wesentlichen Risikofaktor für ein Harnröhrenrezidiv stellt die Ausdehnung des Karzinoms in die prostatische Harnröhre dar, insbesondere wenn das Prostatastroma invadiert wird. Levinson u. Mitarb. [8] konnten zeigen, dass eine sekundäre Urethrektomie bei 6 von 124 Patienten (4,8 %) notwendig war, wobei sich die zugrunde liegende Pathologie überwiegend auf den initialen Tumorbefall der prostatischen Harnröhre und das Vorhandensein eines Carcinoma in situ bzw. eines multifokalen Tumorbefalls der Harnblase verteilte.
