Die Pathophysiologie der chronischen Herzinsuffizienz (CHI) ist in den letzten 15 Jahren zunehmend besser verstanden worden: Die CHI ist nicht mehr nur eine hämodynamische Erkrankung, sondern präsentiert sich als komplexes Bild neurohormonaler, immunologischer und metabolischer Veränderungen. Dabei ist die neurohormonale Aktivierung die derzeitige Grundsäule im pathophysiologischen Verständnis der CHI. Ziel der neuen therapeutischen Konzepte ist es, die Symptomatik zu verbessern und die Mortalität zu reduzieren.

Inhibition proinflammatorischer Zytokine. Eine inflammatorische Immunaktivierung ist charakteristisch für die CHI und trägt zu ihrer Morbidität und Progression bei - TNF-α ist dabei das wesentliche Zytokin und zentraler Mediator dieser Prozesse. Obwohl erste große klinische Studien keinen signifikanten Erfolg der TNF-Inhibition nachweisen konnten, ist das Prinzip der immunmodulatorischen Therapie ein viel versprechendes therapeutisches Konzept. Verschiedene Ansätze zur Inhibition proinflammatorischer Zytokine werden derzeit getestet.

Korrektur einer Anämie. Eine Anämie ist typisch für die CHI und wird doch erst seit kurzem wissenschaftlich genauer untersucht. Der Einfluss von erniedrigtem Hämoglobin auf Symptomatik und Prognose der CHI ist gezeigt worden. Ob die therapeutische Verbesserung einer CHI-bedingten Anämie eine signifikante Verbesserung der Morbidität und/oder Mortalität bewirkt, ist Gegenstand aktueller klinischer Studien.

Inhibition der Xanthinoxidase. Der durch das Enzym Xanthinoxidase (XO) bestimmte Stoffwechselweg des Purinabbaus ist über die Endstrecke der Harnsäurebildung hinaus ein wichtiger Faktor in der Pathophysiologie der CHI. Die Aktivität der XO ist bei CHI durch Wechselwirkungen mit metabolischen (Hypoxie, Insulinresistenz) und immunologischen (Sauerstoffradikale, Zytokine) Störungen erhöht. Die XO-abhängige vermehrte Radikalbelastung ist kausal an der endothelialen Funktionsstörung beteiligt. Die XO-Inhibition mit Allopurinol als therapeutischer Ansatz hat einen positiven Effekt auf die periphere Perfusion, die endotheliale Funktion, die myokardiale Kontraktilität und die myokardiale energetische Effizienz. Weitere Studien sind erforderlich, um den Stellenwert der XO-Inhibition in der CHI-Therapie zu belegen.

Beeinflussung der CHI über den Stoffwechsel. Im Rahmen der CHI treten charakteristische metabolische Veränderungen auf, die einerseits die energetische Effizienz vermindern (Insulinresistenz), andererseits eine katabole/anabole Imbalance bewirken. Metabolische Therapiekonzepte mit dem Ziel, den Energiestoffwechsel (myokardial als auch skelettmuskulär) zu verbessern, werden aktuell getestet. Eine gezielte Therapie gegen Gewichtsverlust und kardiale Kachexie gibt es derzeit nicht.
