Fallvorstellung

Die 56-jährige Patientin stellte sich mit einem seit etwa einem halben Jahr zunehmenden trockenen Husten und Dyspnoe vor. Die Patientin war vorher niemals ernsthaft krank gewesen, rauchte nicht und nahm keine Medikamente ein. In der körperlichen Untersuchung fanden sich außer einer Tachypnoe (20/min) und vereinzelten Rasselgeräuschen über den basalen Lungenabschnitten beidseitig keine pathologischen Befunde. Laborchemisch fielen lediglich ein leicht erhöhtes C-reaktives Protein (39 mg/l, Norm < 20 mg/l) und eine leichte Hypoxämie (PaO2 8,6 kPa, Norm > 10 kPa) auf. Die Lungenfunktionsprüfung zeigte eine leichte Restriktion und eine leichtgradige Einschränkung der Diffusionskapazität. Zur weiteren Abklärung der Symptomatik wurde eine CT des Thorax mit hochauflösenden Schichten (HRCT) (Abb. [1]) sowie anschließend eine Bronchoskopie mit transbronchialer Biopsie des Lungengewebes durchgeführt (Abb. [2]).
