Zusammenfassung

Zielsetzung: Die Effizienz der Pharmakotherapie der Schizophrenie mit Ziprasidon wurde im Rahmen einer Kosten-Effektivitäts-Analyse mit den Alternativen Haloperidol, Risperidon und Olanzapin verglichen. Methodik: Die Schizophreniebehandlung wurde über einen Zeithorizont von einem Jahr mittels eines Markov-Modells simuliert. Effektivitätsparameter war die rückfallfreie Zeit in Monaten. Es wurden neben der Wirksamkeit der Therapiealternativen auch deren unerwünschte Arzneimittelwirkungen (UAW) Akathisie, andere extrapyramidale Symptome (EPS), Gewichtszunahme und Prolaktin-abhängige Nebenwirkungen im Modell berücksichtigt. Der Zusammenhang zwischen dem Auftreten von UAWs und dem Auftreten von Rückfällen infolge von Noncompliance war zentraler Bestandteil des Modells. Primär wurde die Perspektive der Gesetzlichen Krankenversicherung eingenommen. Darüber hinaus wurde auch die gesundheitspolitische Perspektive berücksichtigt. Ergebnisse: Ziprasidon zeichnet sich durch eine längere rückfallfreie Zeit sowie geringere Gesamt- und effektivitätsadjustierte Kosten aus. Die effektivitätsadjustierten direkten Kosten sind für Patienten unter Ziprasidon-Therapie niedriger als bei den anderen Alternativen (Ziprasidon 2370 €, Haloperidol 3054 €, Risperidon 2456 €, Olanzapin 2514 €, pro rückfallfreien Monat). Schlussfolgerung: Die Schizophrenietherapie mit Ziprasidon ist unter den hier getroffenen Annahmen sowohl aus der Perspektive der Gesetzlichen Krankenversicherung als auch aus der gesundheitspolitischen Perspektive die effizienteste der betrachteten Alternativen.

Abstract

Aim: The aim of this economic evaluation was to compare the cost-effectiveness of ziprasidone with haloperidol, risperidone, and olanzapine for treating schizophrenia. Methods: Treatment of schizophrenia was modelled over one year by means of a Markov model. Common adverse drug reactions (ADR), including akathisia, other extrapyramidal symptoms (EPS), weight gain and prolactin-related side effects were considered. The model further took into account non-compliance due to the occurrence of ADR, which eventually can lead to relapse of schizophrenic patients. The effectiveness was measured as the number of months a patient is free of psychotic symptoms. Primary, the model took the perspective of the compulsory health insurance funds. Secondary the health policy perspective was taken. Results: Treatment with ziprasidone resulted in higher effectiveness and lower costs. Accordingly, effectiveness-adjusted direct costs for patients treated with ziprasidone were lower than those for patients treated with the other alternatives (ziprasidone 2.370 €, haloperidol 3.054 €, risperidone 2.456 €, olanzapine 2.514 € per month of controlled symptoms). Conclusions: In this model, treatment of schizophrenia with ziprasidone was the most effective of considered alternatives from the perspective of the compulsory health insurance funds and from health policy perspective.
