Zusammenfasssung

Selbstexpandierende Stents sind geeignet die rasche Beseitigung einer Darmobstruktion zu erreichen. Alternative Behandlungsmethoden sind die Bypasschirurgie oder die kontinuierliche äußere Drainage. Es gibt zunehmende Hinweise, dass das Stenting sowohl unter den Gesichtspunkten der Kosteneffektivität als auch der Wirksamkeit die bevorzugte Methode zur Palliation maligner Magenausgangsstenosen ist. Die Autoren beschreiben die Durchführung der Prozedur und die notwendige Ausrüstung. Sie zeigen die Indikationen, die Erfolgsrate und die Grenzen der Methode auf. Weitere Untersuchungen, die die Vorzüge der laparoskopischen chirurgischen Technik und verbesserte Stentausrüstungen in Rechnung stellen sind nötig, um die Vorteile des Stenting für Patienten mit einer längeren Lebenserwartung zu prüfen. Patienten mit einem gastroduodenalen Stent erfordern häufig zusätzliche Interventionen, entweder wegen eines Verschlussikterus oder wegen eines sekundären Stentversagens. Eine engmaschige Nachsorge ist erforderlich, bei der eine gute Zusammenarbeit zwischen Endoskopikern, Radiologen und im onkologischen Team erforderlich ist.

Abstract

Self-expanding stents are designed to provide quick relief of enteric obstruction. Alternatives consist of bypass surgery or continuous external drainage. Evidence is emerging that stenting is the preferred method of palliation of malignant gastric outlet obstruction, both in terms of cost-effectiveness and efficacy the authors describe the procedure and the equipment which is necessary for enteral stenting. They are pointing out the indications, the success rate and the limitations of the method. Further trials, which take into account advances in laparoscopic surgical technique and improved stent design, are needed to confirm these advantages for patients with a longer life expectancy. Patients with a gastroduodenal stent frequently require further intervention, either for late obstructive jaundice or stent failure, and close follow-up is mandatory as well as good collaboration between endoscopists, radiologists, and the cancer team.
