Zusammenfassung

30 extrahierte untere Prämolaren wurden nach endodontischer Vorbehandlung in 6 Gruppen zu je 5 Zähnen verschiedenartig restauriert. Die Zähne wurden nach thermischer und mechanischer Vorbelastung einem Bruchlastversuch in einer Zwick-Apparatur ausgesetzt. Die Bruchkräfte wurden gemessen und die Frakturhöhe relativ zur Wurzel- bzw. Kronenlänge ausgewertet. Dabei wiesen gegossene Stiftaufbauten eindeutig die geringste Frakturresistenz auf. Die statistische Auswertung mittels ANOVA (a = 0,05, Post hoc test Fisher's PLSD) zeigte zudem signifikante Unterschiede hinsichtlich der Frakturlokalisation. Zähne mit Stiftversorgung neigten zu Frakturen im Bereich des apikalen Wurzeldrittels und Zähne ohne Stiftversorgung zu Frakturen im zervikalen bzw. koronalen Bereich.

Die In-vitro-Studie zeigte, dass Stifte Zahnwurzeln nicht stabilisieren, sondern nur zur Retention von Restaurationen bei Zähnen mit großem Substanzverlust herangezogen werden sollten.

Summary

The aim of this study was to analyze the influence of different restorations with or without posts on the fracture resistance of endodontically treated and reconstructed premolars. 30 extracted lower premolars were restored with 6 different methods: Mod-composite filling (F) (Admira-, Voco, Germany); Ti-post (ER, Komet, Germany) with composite core and full metal crown (KSP); partial metal crown (TK); Ti-post with cast metal core and metal crown (KSG); ceramic endo-crown (EK) (Wolceram); ceramic inlay (KI). The teeth were stressed by thermo-cycling (5000 times; 5 °C-55 °C), embedded in a pseudo-realistic model, and subjected to a long-term occlusal loading procedure (30,000 cycles; 50 N at random in axial and in both 45° oral and vestibular direction). Following this, the teeth were fractured by a Zwick device (45° vestibular direction, feed rate 1 mm/min). Fracture resistance and fracture line were determined. The mean values of fracture resistance were: F (1021 ± 110 N); KSP (873 ± 201 N); TK (1040 ± 67 N); KSG (586 ± 82 N); EK (823 ± 192 N); KI (839 ± 350 N). One-way ANOVA (a = 0.05, Post hoc test according to Fisher's PLSD) revealed significant differences between F/KSG (p = 0.002), TK/KSG (p = 0.001), KI/KSG (p = 0.050) and KSP/KSG (p = 0.027). Statistical analysis also shows significantly differences for the location of the fracture line. Teeth restored with posts fractured in the middle of the root and without posts in neck region. Metal post with cast core reconstructions show significantly decreased fracture resistance compared to all other restorative measures of the teeth studied.
