Zusammenfassung

Fragestellung: Ziel der vorliegenden retrospektiven Untersuchung war es, krankheitsfreies Überleben, Gesamtüberleben, lokale Tumorkontrolle und die Fernmetastasierungsrate bei Patientinnen mit einem Mammakarzinom nach brusterhaltender Operation und Strahlenbehandlung zu überprüfen. Patientinnen und Methoden: Insgesamt wurden 274 Patientinnen mit einem Mammakarzinom zwischen 1990-1997 in unserer Klinik behandelt und analysiert. Die gesamte Brust wurde homogen bestrahlt (2,0 Gy bis 50 Gy), gefolgt von einem Boost von 10-16 Gy. Die mittlere Nachbeobachtungszeit betrug 55 Monate. Gesamtüberleben, lokale Tumorkontrolle und Fernmetastasierungsrate wurden anhand der klinischen Daten und Nachuntersuchungen erhoben und analysiert. Ergebnisse: Das krankheitsfreie Gesamtüberleben betrug nach fünf Jahren 93 %. 18 Patientinnen (7 %) entwickelten 3 bis 60 Monate nach Therapie ein Lokalrezidiv. Bei 24 (9 %) Frauen lag ein kontralaterales Karzinom vor. Die Überlebensrate nach Lokalrezidiv betrug 78 % (5 Jahre) bzw. 20 % (7 Jahre). Hinsichtlich der Lokalrezidivrate erwiesen sich folgende Faktoren unabhängig als signifikant: Fernmetastasierung, Rezeptorstatus, Lymphangiosis und Operationsmodus. Eine Fernmetastasierung entwickelte sich bei 9 % der Patientinnen, hauptsächlich Knochenmetastasen (71 %). Die Überlebensrate nach Fernmetastasierung betrug 64 % (5 Jahre) bzw. 10 % (8 Jahre). Tumorgröße, Grading, Rezeptorstatus, Lymphangiosis und Vorliegen eines Lokalrezidives führten signifikant häufiger zu einer Fernmetastasierung. Schlussfolgerung: Die Ergebnisse nach brusterhaltendem Vorgehen bei einem unselektionierten Kollektiv von Patientinnen mit einem Mammakarzinom entsprechen den publizierten Daten hinsichtlich Gesamtüberleben, lokaler Tumorkontrolle und Fernmetastasierungsrate.

Abstract

Objective: To evaluate mono-institutional results concerning tumor free survival, overall survival, local tumor control and rate of distant metastasis following breast-conserving therapy. Patients and methods: Retrospectively, 274 breast cancer patients who were treated between 1990-1997 in our institution were analysed. The whole breast was homogeneously irradiated (2.0 Gy to 50 Gy), followed by a boost of 10-16 Gy to the tumor bed. Mean follow-up was 55 months. Overall survival, local tumor control and rate of distant metastasis were analysed. Results: Cause-specific survival at 5 years after treatment was 93 %. Within 3 to 60 months following treatment, 18 (7 %) patients suffered from ipsilateral breast recurrence. 24 (9 %) patients developed contralateral carcinoma. Survival from local recurrence (single manifestation) was 78 % at 5 years after treatment, 20 % at 7 years. Occurrence of local failures was significantly correlated to receptor status, contralateral carcinoma, distant metastasis and surgical technique and not to tumor size, margins, grading, nodal status, age or lymphangiosis. 9 % of the patients developed distant metastases, predominantly bone metastases (71 %). Survival from distant metastasis was 64 % at 5 years, 10 % at 7 years. Occurrence of distant metastasis was significantly correlated to grading, tumor size, receptor status, lymphangiosis or local recurrence. Conclusion: Our institutional results show that tumor free survival, overall survival, local tumor control and distant failure rate achieved by breast conserving therapy are within the range of literature data.
