Änderungen bei Tumoren des Gastrointestinaltraktes in der 6. Auflage der TNM-Klassifikation

Zusammenfassung

Die Änderungen in der 6. Auflage (2002) der TNM-Klassifikation maligner Tumoren des Verdauungstraktes werden dargestellt und kommentiert. Die allgemeinen Regeln der TNM-Klassifikation sind für die Tumoren des Verdauungstraktes nicht geändert worden. Erwähnt werden die Untersuchungen und Klassifikation von Schildwächterlymphknoten und isolierten Tumorzellen. Die jeweiligen Änderungen für die einzelnen Tumorentitäten werden hinsichtlich der Definition regionärer Lymphknoten, der T-, N- und M-Kategorien sowie der Stadiengruppierung aufgezeigt. Die Änderungen werden kommentiert und in Bezug auf ihre klinische Bedeutung erläutert. Schlussendlich wird auf das geänderte Verfahren der UICC hingewiesen, mit dem die zunehmende Komplexität der Daten und verfahrenstechnische Schwierigkeiten für zukünftige Änderungen der Klassifikationen besser berücksichtigt werden sollen. Die bisher benutzten Prozeduren zur Etablierung von Veränderungen der TNM-Klassifikation können nicht weiter verwendet werden. Die Forderung, Änderungen durch angemessene, EBM-basierte Entscheidungen zu unterlegen, begründet eine Neustrukturierung der bisherigen Entscheidungsprozesse.

Abstract

The changes in the 6^th edition of the TNM Classification of tumours of the digestive system will be presented. The general rules for the tumours of the digestive system have not been changed. The examination and classification of sentinel lymph nodes and isolated tumour cells (ITC) will be mentioned. Specific changes for the tumour entities as to the definitions of the regional lymph nodes, and the T-, N-, and M categories will be presented. Changes will be supplemented by a commentary and an evaluation as to their clinical importance. Finally, we will comment an the changed procedure of the UICC to deal with upcoming proposals for changes of the TNM classification. It has become evident that the former procedures to evaluate and establish proposals for changes are no longer acceptable and should be replaced by a system, which considers evidence based mechanisms as the basis of upcoming changes. This will be accompanied by an ongoing literature review to timely papers with relevant proposals for being considered in the TNM system.
