Zusammenfassung
Peak-flow-Meter sind für Asthma-Patienten wertvolle Instrumente, um einen objektiven Zugang zur Krankheit zu finden, die Symptomwahrnehmung zu schulen und den schwankenden Charakter der Erkrankung zu erkennen. Zur alleinigen Therapiesteuerung eignen sich die Peak-flow-Werte mithilfe des Ampelschemas jedoch nicht, da sie häufig den Asthmaschweregrad unterschätzen und Exazerbationen unzureichend erkennen. Insbesondere kann es fatal sein, den grünen Bereich mit „Alles in Ordnung” zu interpretieren und damit seine Wertigkeit zu überschätzen. Im Vordergrund des Selbstmanagements des Asthma bronchiale müssen daher die Symptome stehen. Die korrekte Durchführung der Peak-flow-Messung und die möglichen Konsequenzen für das Selbstmanagement erfordern eine intensive Schulung der Patienten und möglichst individualisierte, zwischen Arzt und Patient abgestimmte Ampelschemata und Aktionspläne.
Summary
Assessment of peak flow rates may help asthmatic patients to recognize changes in asthma severity. A self-management plan based only on peak flow rates is nevertheless inadequate, as it often underestimates the really degree of an exacerbation. Especially it may be fatal to believe, in the „green zone” of peak flow rates everything is ok. The symptoms are more important to monitor asthma and to control the disease by a self-management plan. Patient education is essential for correct assessment and self-management. Perhaps individualized peak flow levels designated for changes in treatment may provide a better support for asthma management by peak flow rates.
