Die Schizophrenie stellt eine häufige Erkrankung mit einer Lebenszeitprävalenz von ca. 1 % dar, die sich bereits im frühen Erwachsenenalter manifestiert und wesentliche Bereiche des Erlebens beeinträchtigt. Neben der oft mehr ins Auge fallenden Positivsymptomatik stellt die Ausprägung der Negativsymptomatik einschließlich kognitiver Defizite den entscheidenden Prädiktor für die weitere Prognose dar. Bei einem Teil der Patienten (ca. 20 bis 35 %) kommt es zu einer chronischen Verlaufsform mit Ausbildung eines Residualzustandes, welcher sich in der Beeinträchtigung verschiedener Aspekte der selbständigen Lebensführung äußern kann und hohe direkte sowie indirekte Krankheitskosten verursacht. Trotz der großen klinischen Relevanz sind die ätiopathogenetischen Grundlagen dieser Erkrankung bisher nur in Ansätzen verstanden. In diesem Beitrag sollen die wichtigsten Aspekte zur Epidemiologie, Ätiopathogenese, Symptomatologie, Verlauf, Diagnostik und Differenzialdiagnostik der schizophrenen Erkrankung dargestellt werden.
