Die Rekonstruktion der Extensor pollicis longus-Sehne mit einem Sehneninterponat

Zusammenfassung

Zur Therapie der Ruptur der Sehne des M. extensor pollicis longus (EPL) stehen als operative Verfahren die Sehnenrekonstruktion unter Verwendung eines freien Sehneninterponates oder motorische Ersatzoperationen zu Verfügung.

In einer retrospektiven Studie sollten die Ergebnisse der Sehnenrekonstruktion mit einem Sehneninterponat überprüft werden.

In den Jahren 1992 bis 1998 wurde bei 58 Patienten eine Rekonstruktion des EPL unter Verwendung eines freien Sehnentransplantates von der Palmaris longus-Sehne durchgeführt. 48 Patienten konnten nach durchschnittlich 31 Monaten nachuntersucht werden. Es handelte sich um 25 Frauen und 23 Männer mit einem durchschnittlichen Alter von 49 Jahren. Bei 19 Patienten war es nach einer distalen Radiusfraktur zu einer subkutanen Ruptur gekommen, bei sieben Patienten führte eine Schnittverletzung zur Durchtrennung des EPL, und bei 22 Patienten fand sich eine subkutane Ruptur ohne erinnerliches Trauma. Die Rekonstruktion der Sehne fand in einem Zeitraum zwischen einer und 40 Wochen nach dem Trauma statt. Unter Verwendung des Beurteilungsschemas von Geldmacher und Köckerling fanden sich im Durchschnitt gute Ergebnisse. Bei fünf Patienten wurde das Ergebnis der Operation mit sehr gut, bei 35 mit gut und bei acht mit befriedigend bewertet. Entsprechend diesem Beurteilungsschema fand sich kein schlechtes Ergebnis. Die statistischen Analysen zeigten, dass diese Ergebnisse unabhängig vom Lebensalter der Patienten, dem Zeitintervall zwischen Ruptur der Sehne und Sehnenrekonstruktion sowie der Ursache der Sehnenruptur erzielt wurden.

Der durchschnittliche Punktewert des DASH-Score betrug 14 (0 bis 63). Unter Berücksichtigung unserer klinischen Ergebnisse kann durch die sekundäre Rekonstruktion der EPL-Sehne unter Verwendung eines Sehnentransplantates eine gute Funktion des Daumens, unabhängig vom Alter der Patienten und der Ursache der Sehnenruptur, zumindest bis zu einem Zeitraum von 40 Wochen nach der Ruptur wiederhergestellt werden. Als Vorteile gegenüber der motorischen Ersatzoperation mit Transposition der Sehne des M. extensor indicis sehen wir, dass eine Umlernungsphase entfällt und keine zusätzliche motorische Einheit geopfert werden muss. Es verbleibt daher eine unangetastete isolierte, kraftvolle Streckung des Zeigefingers.

Wir empfehlen die sekundäre EPL-Sehnenrekonstruktion daher insbesondere bei Patienten, deren berufliche Tätigkeit oder Hobby eine isolierte kräftige Zeigefingerstreckung erforderlich macht (Chirurgen, Musiker), oder bei Patienten mit zusätzlichen Strecksehnenverletzungen, die eine Sehnentransposition nicht ermöglicht.

Abstract

To restore function of the ruptured extensor pollicis longus (EPL) tendon, two operative principles have been described: reconstruction of the original tendon using a tendon interposition graft or a tendon transposition operation, most commonly transposition of the extensor indicis.

From 1992 to 1998, we reconstructed the extensor pollicis longus tendon using an interposition graft in 58 patients. 48 patients were available for follow-up after a mean of 31 months. The mean age of the 25 women and 23 men was 49 years. The time between tendon rupture and reconstruction ranged from one to 40 weeks. The tendon rupture occurred in 19 patients following a radius fracture, in 22 patients without history of trauma and in seven patients it was due to a sharp injury.

Using the clinical score of Geldmacher and Köckerling, five patients achieved a very good, 35 patients a good and eight patients a satisfactory result. No patient had a poor result. According to the statistical analyses, these results were independent of the age of the patients, the origin of the rupture or the time interval between rupture and tendon reconstruction. The average DASH score measured 14.

Considering our results, secondary reconstruction of the original EPL tendon appears to be a valuable alternative to the commonly performed tendon transpositions. It yields good clinical results with little remaining functional impairment and without the need to sacrifice another functional motor. We strongly recommend this technique in patients who require an independent and strong extension of the index finger in their profession (surgeons, musicians) and in patients with severe trauma that does not allow the sacrifice of another intact motor unit.
