Zusammenfassung

Fragestellung

Die totale mesometriale Resektion ist charakterisiert durch (1) die En-bloc-Resektion des Uterus, der proximalen Vagina und des Mesometriums als einer aus der Embryonal- und Fetalentwicklung abgeleiteten Gewebeeinheit, (2) die Absetzung der rektouterinen subperitonealen Bindegewebsverdichtungen unmittelbar oberhalb des freigelegten Plexus hypogastricus inferior und (3) die erweiterte pelvine/paraaortale Lymphknotendissektion unter Erhalt des Plexus hypogastricus superior.

Patienten und Methoden

Die operative Therapie des Zervixkarzinoms der FIGO-Stadien IB, IIA und in ausgewählten Fällen IIB mit TMMR wird seit 7/1998 in einer kontrollierten prospektiven klinischen Studie mit den Endpunkten Tumorkontrolle und behandlungsbedingte Morbidität evaluiert. Bis 12/2002 wurden 78 Patientinnen, die an Zervixkarzinomen der Stadien pT1b1 (n = 52), pT1b2 (n = 10), pT2a (n = 3), pT2 b (n = 13) erkrankt waren, mit TMMR ohne adjuvante Strahlentherapie behandelt.

Ergebnisse

52 % der Patientinnen wiesen histopathologische Risikofaktoren auf. Nach einer medianen Nachbeobachtungszeit von 26 (4 - 57) Monaten kam es bei insgesamt 6 Patientinnen zum Tumorrückfall. Zwei Patientinnen hatten Lokalrezidive, bei zwei Patientinnen traten gleichzeitig lokoregionäre Rezidive und Fernmetastasen auf, bei zwei Patientinnen waren zum Zeitpunkt des Tumorrückfalls nur Fernmetastasen nachweisbar. Drei Patientinnen sind an ihrer Krebserkrankung verstorben. Komplikationen der Schweregrade 1 und 2 ereigneten sich 21- bzw. 5-mal, keine Patientin entwickelte Grad-3- oder -4-Komplikationen. Schwere Beeinträchtigungen der Urethrovesikal-, Vulvovaginal- oder Anorektalfunktionen wurden nicht festgestellt.

Schlussfolgerung

Die bisherigen Ergebnisse der totalen mesometrialen Resektion bei der Behandlung von Patientinnen mit Zervixkarzinom der FIGO-Stadien IB und IIA sind günstig. Aufgrund der hohen Wahrscheinlichkeit der Tumorkontrolle und der minimalen behandlungsbedingten Morbidität kann eine Verbesserung des therapeutischen Index erwartet werden.

Abstract

Purpose

Total mesometrial resection (TMMR) is characterized by (i) the en bloc resection of the uterus, proximal vagina and mesometrium as a developmentally defined entity, (ii) transection of the rectouterine dense subperitoneal connective tissue above the level of the exposed inferior hypogastric plexus, and (iii) extended pelvic/periaortic lymph node dissection preserving the superior hypogastric plexus.

Patients and Methods

Since 7/1998 we prospectively studied the outcome in patients treated with TMMR for cervical carcinoma FIGO stages IB, IIA and selected IIB with respect to tumor control and treatment-related morbidity. By 7/2002 78 patients with cervical cancer stages pT1b1 (n = 52), pT1b2 (n = 10), pT2a (n = 3), pT2 b (n = 13) underwent TMMR without adjuvant radiation.

Results

52 % of the patients exhibited histopathological high risk factors. At a median observation period of 26 months (4 - 57 months) two patients relapsed locally, two patients developed pelvic and distant metastases and two patients had only distant recurrences. Three patients died from disease. Grade 1 and 2 complications occurred 21 resp. 5 times, no patient had grade 3 or 4 complications. No severe long-term impairment of pelvic visceral functions related to autonomic nerve damage was detected.

Conclusion

Based on these preliminary results TMMR has a promising therapeutic index with a high probability of locoregional control at minimal short and long-term morbidity.
