Systemische Mastozytose als Ursache einer Osteoporose

Zusammenfassung

Das Syndrom der Mastozytose reicht von der isolierten Hautmanifestation (Urticaria pigmentosa) über die systemische Mastozytose bis hin zu der extrem seltenen Form der Mastzellenleukämie. Bei der systemischen Mastozytose sind lokalisierte (osteolytische-osteosklerotische Herde) und/oder generalisierte (Osteoporose-Osteosklerose-)Manifestationen in bis zur Hälfte der Fälle anzutreffen.

In der dargestellen Kasuistik stellt eine Osteoporose interessanterweise das Erstsymptom der Mastozytose dar. Nach kurzer Darlegung der pathologisch-anatomischen Grundlagen wird ein Überblick über Haut- und Knochenbefunde bei der Mastozytose und den derzeitigen Therapieoptionen gegeben.

Abstract

The syndrome of mastocytosis extends from cutaneous urticaria pigmentosa via systemic mastocytosis to the rare form of mast cell leukaemia. In systemic mastocytosis bone lesions occur in about 50 % of the cases. These lesions can be localised (osteolytic-osteosclerotic foci) or generalised (osteoporosis-osteosclerosis). It is interesting to note that osteoporosis was the first symptom of systemic mastocytosis in the presented case report. After a brief report on the pathogenetic mechanism, an update overview on findings of the skin and bones in mastocytosis is given and present therapeutic options are explained.
