Umstellung von hochdosiertem Methadon (Methadon Racemat®) auf Buprenorphin (Subutex®) im Rahmen ambulanter Opiatsubstitution

Zusammenfassung

Die Literatur empfiehlt die Umstellung auf Buprenorphin bei einer stabilen Methadondosis von 30-40 mg/d. Diskutiert wird auch die Umstellung bei höherer Methadondosis auf Buprenorphin. Zurzeit existieren wenige Mitteilungen aus der Praxis zur Hochdosisumstellung. Im Folgenden wird ein Fallbeispiel geschildert, welches das Vorgehen bei Umstellung von hochdosierten Methadonpatienten (80 mg/d) auf Buprenorphin beschreibt.

Abstract

Current literature recommends the conversion to buprenorphine in patients treated with a stable methadone dose of 30-40 mg/d. However, few reports on buprenorphine conversion in patients under high-dose methadone exist. The following case example describes the procedure for conversion of high-dose methadone (80 mg/d) to buprenorphine.
