Choristom des Zwerchfells

Zusammenfassung

Tumoren ausgehend vom Zwerchfell sind selten und aufgrund der speziellen Lokalisation schwer zu diagnostizieren. Vorgestellt wird der Fall eines 31-jährigen Mannes mit einem Choristom des linken Zwerchfells, dessen Lokalisation und Genese erst durch anterolaterale Thorakotomie geklärt werden konnte. Sowohl eine vorangegangen CT-gesteuerte Punktion wie auch eine diagnostische Laparoskopie konnten trotz ausreichender Materialgewinnung für den Pathologen keine sichere Diagnose stellen. Anhand des vorliegenden Falles wird der Stellenwert der Diagnostik zur Auswahl des geeigneten operativen Zugangsweges - in diesem Fall der anterolateralen Thorakotomie - und die Versorgung desselben dargestellt.

Abstract

Tumors of the diaphragm are rare and caused by there special localisation difficult to diagnose. We describe the case of a 31 year old man with a choristoma of the left diaphragmal crux where the localisation and genesis could only be cleared through lateral thoracotomy. Nor the CT guided puncture or the laparoscopy-although sufficient samples for the pathology could be gained-allowed the right diagnose. The importance of good diagnostic approach to select the right operative procedure-in this case the anterolateral thoracotomy-is described.
