Continuous Intraoperative Intraperitoneal Hyperthermic Chemoperfusion (CIIPHCP) in the Treatment of Advanced Stage and Recurrent Granulosa Cell Tumors. Report of Two Cases

Abstract

Background: Treatment of advanced stages and recurrent ovarian granulosa cell tumors, has not been established yet. The effectiveness of radiation therapy could not be proven. Systemic chemotherapy has shown promising results, but with severe side effects and high incidence of relapse.
Case Reports: We report of one patient with advanced stage III C, and one patient with bulky recurrent ovarian granulosa cell tumors. Both patients were treated with a combination of surgical debulking, Continuous Intraoperative Intraperitoneal Hyperthermic Chemoperfusion (CIIPHCP) with Cisplatin and one of them with adjuvant systemic chemotherapy.
Conclusion: CIIPHCP appears to offer a promising procedure in addition to surgical debulking and systemic chemotherapy for treatment of advanced or recurrent ovarian granulosa cell tumors. The present report is the first concerning the question of adding Intraoperative Hyperthermic Chemoperfusion in the treatment of advanced or recurrent ovarian granulosa cell tumors.

Zusammenfassung

Einführung: Die Therapie der fortgeschrittenen Stadien und der rezidivierenden Granulosa-Zelltumoren ist noch nicht etabliert. Die Effektivität der Strahlentherapie konnte nicht eindeutig bewiesen werden. Systemische Chemotherapie erzielte vielversprechende Ergebnisse, wurde aber begleitet von ausgeprägten Nebenwirkungen und einem hohen Prozentsatz an Rezidiven.
Fallberichte: Wir berichten über eine Patientin im fortgeschrittenem Stadium III C und über eine weitere Patientin mit ausgedehntem rezidiverenden Granulosa-Zelltumor. Beide Patientinnen wurden mit einer Kombination von chirurgischer Tumorreduktion und Kontinuierlicher Intraoperativer Intraperitonealer Hyperthermischer Chemoperfusion (CIIPHCP) mit Cisplatin behandelt.
Schlussfolgerung: CIIPHCP scheint einen viel versprechenden therapeutischen Zusatz zur tumorreduktiven Operation und der adjuvanten systemischen Chemotherapie der fortgeschrittenen Stadien und der rezidivierenden Granulosa-Zelltumoren zu bieten. Dieser Bericht ist der Erste, der die Frage über das mögliche Einsetzen der CIIPHCP in der Therapie der fortgeschrittenen Stadien und der rezidivierenden Granulosa-Zelltumoren stellt.
