Zusammenfassung

Diese Übersichtsarbeit liefert Kriterien für die Diagnose von erhöhten Strömungsgeschwindigkeiten der basalen Hirngefäße. Anhand der Anamnese, der Klinik und der erhobenen TCD-Befunde werden die Differenzialdiagnosen diskutiert und aufgezeichnet. Diese Übersichtsarbeit umfasst die Evaluation von Stenosen, AV-Malformationen und Fisteln, intrakraniellen Kollateralisationen sowie Befundkonstellationen bei SAB, Meningitiden, Vaskulitiden. Weiterhin wird die Differenzialdiagnose der zerebralen Hyperämie nach Rekanalisation nach embolischem Verschluss, beim Präeklampsie-/Eklampsie-Syndrom sowie bei metabolischen Störungen wie Anämie, MELAS und Hypoglykämie diskutiert.

Abstract

This review offers criteria to diagnose pathologically elevated intracranial blood flow velocities. According to medical history, clinical signs and symptoms and TCD findings differential diagnoses are discussed. These include stenoses of arteries, AV-malformations and fistulas, collateration ways as well as findings in patients with aneurysmatic and traumatic subarachnoid hemorrhage, meningitis and vasculitis. Furthermore, the broad differential diagnosis of cerebral hyperemia seen after recanalisation, in pre-eclampsia/eclampsia syndrom, and metabolic disorders as anemia, MELAS and hypoglycemia is discussed.
