Psychoanalytische Überlegungen zur bipolaren affektiven Erkrankung - Neues seit Abraham?

Zusammenfassung

Dieser Beitrag gibt eine Übersicht über psychoanalytische Beiträge zu bipolaren affektiven Psychosen. Dabei wird von den frühen Arbeiten Freuds und Abrahams ausgegangen, im Anschluss Ich-psychologische, Objektbeziehungstheoretische und kleinianische Beiträge dargestellt. Es wird die These vertreten, dass einige Fragestellungen, insbesondere die Rolle von Über-Ich und Introjektionsvorgängen, die Bedeutung narzisstischer Phänomene und die ätiopathogenetische Bedeutung frühester Beziehungsstörungen im Laufe der Jahrzehnte ausdifferenziert wurden. Eine ausführliche Fallbeschreibung aus der Literatur wird dargestellt und diskutiert. Abschließend werden Bedingungen für die analytische Behandlung dargestellt.

Abstract

An overview on psychoanalytic contributions to the understanding of bipolar affective psychoses is given. Starting with Freud and Abraham, contributions from Ego-psychology, object-relational-theory and the Kleinian school are outlined. The view is taken that several questions, particularly the role of the super-ego and of narcissistic phenomena, as well as the pathogenetic significance of earliest relationships, have been differentiated over the decades. A detailed case-description from the literature is presented and discussed. Finally conditions for psychoanalytic treatment are described.
