Etofenamat-Kontaktallergie

Zusammenfassung

Nach Behandlung einer Knieverletzung im Urlaub (Sturz vom Fahrrad) mit einem nichtsteroidalen Antiphlogistikum in Sprayform, entwickelte eine 48-jährige Hauswirtschafterin beim erneuten Aufbringen ein dreiviertel Jahr später eine Kontaktdermatitis mit Streuherden. Da die Spraydose nach Gebrauch verworfen worden war, konnte erst wesentlich später der Wirkstoff Etofenamat als Kontaktallergen entlarvt werden. Um den ätiologischen Zusammenhang zu beweisen, besprühte die Patientin ihr Knie mit dem ein Jahr später aus Spanien mitgebrachten Externum ein weiteres Mal und stellte sich zur Demonstration mit dem Rezidiv des Ekzems in der Klinik vor.

Abstract

During holidays on a Spanish island a 48-year old housekeeper fell from her bycicle and hurt her right knee. The injury was treated by a local doctor with a spray containing a non-steroidal antiinflammatory drug. Eight months later she used the same spray again, because the knee began to hurt after athletic activities. She soon developed eczematous lesions around the knee which spread to the thigh and both arms. As the spray had been thrown away it took some time to disclose its composition. The responsible allergen was identified as etofenamate (1 % in petr.) giving a +++-plus reaction in the 72 hour reading. In order to prove this spray as the causative agent she bought a new can the next year when spending her holidays on the island. Then she treated her knee once more. Visiting the clinic she was capable of demonstrating the offending spray as the source of her allergic contact dermatitis.
