Zusammenfassung
Die ektope Gravidität in einer Sektionarbe ist eine seltene und potenziell lebensbedrohliche
Form der extrauterinen Gravidität. Allgemein gültige Therapieempfehlungen zu dieser
Sonderform existieren nicht. Im beschriebenen Fall wurde primär eine intravenöse Methotrexattherapie
mit dem Ziel gewählt, die schwangerschaftsassoziierte Hyperämie zu reduzieren und
eine sekundäre operative Korrektur der Läsion vorzunehmen. Trotz fallender β-HCG-Werte
kam es 33 Tage nach Methotrexatgabe zu einer hämodynamisch wirksamen Blutung, die
ein operatives Eingreifen notwendig machte. Eine Kombination aus einer initialen Methotrexattherapie
und einer frühzeitigen operativen Korrektur der Sektionarbe wäre sinnvoll gewesen.
Das therapeutische Vorgehen wird zusammen mit den wenigen Fallberichten erörtert.
Abstract
The ectopic pregnancy within a scar of a former caesarean section is a very unusual
and potentially life threatening form of the extrauterine gravidity. For treatment
of such rare cases general guidelines are missing. In the case of this report, a primary
intervenous application of MTX was chosen in order to reduce hyperemia associated
with pregnancy. The treatment should have been followed by a secondary surgical removal
of the lesion. Although β-hCG levels declined a severe persistent haemorrhage occurred
33 days after administration of MTX that made immediate surgical intervention necessary.
Thus, it appears that a combined therapy of initial MTX application and an early surgical
extirpation of the lesion would have been the appropriate treatment. In addition to
the therapeutic procedure, few case reports will be discussed.
