Zusammenfassung

Studienziel: Neben der Röntgennativdiagnostik findet zur Darstellung der Osteochondrosis dissecans (OD) die Magnetresonanztomographie (MRT) zunehmend Anwendung, wobei bezüglich der notwendigen Diagnostik und deren Interpretation keine Einigkeit herrscht. Ziel der vorliegenden prospektiven Studie war, ein diagnostisches Konzept für die OD zu erarbeiten, welches auch im klinischen Alltag einfach und schematisiert angewendet werden kann. Methode: Bei 90 Patienten mit einer OD am Knie- oder Sprunggelenk wurde vor Durchführung einer arthroskopischen Diagnostik eine MRT-Diagnostik (T1- und T2-SE) vorgenommen. Die MRTs wurden anhand des Erscheinungsbildes der Zone zwischen osteochondralem Herd und Mausbett einer 2-stufigen Klassifikation („stabil” oder „instabil”) zugeordnet und mit den arthroskopischen Befunden verglichen. Ergebnisse: Anhand dieser 2-stufigen MRT-Klassifikation konnte in 92 % eine korrekte Vorhersage bezüglich der Stabilität der OD getroffen werden. Bei 7/90 Patienten (8 %) wich die präoperative kernspintomographische Stabilitätsdiagnostik von der arthroskopischen Diagnostik ab, wobei die fehlerhaften MRT-Befunde nur instabile Herde betrafen. Schlussfolgerung: Die gewählte 2-stufige MRT-Klassifikation ist für eine stadienabhängige Therapieplanung sehr gut geeignet.

Abstract

Aim: For the diagnosis of osteochondritis dissecans (OD) MRI is used besides X-ray. However, there is no consensus about the necessity of MRI or the interpretation of MRI. The aim of this prospective study was to find a diagnostic concept for OD which is simple and practicaple for daily routine. Method: In 90 patients with OD of the knee or ankle MRI (T1- and T2-w SE) was carried out before arthroscopy. According to the interface between the osteochondral fragment and the parent bone, MRI was classified in 2 stages (“stable” or “unstable”) and compared with the arthroscopic findings. Results: With the 2-stage classification of MRI, a prediction of the stability of the osteochondral fragment was possible in 92 %. Out of the 90 patients 7 (8 %) differed regarding preoperative stability in MRI compared to stability in arthroscopy. Incorrect diagnosis in MRI was only found among the patients with unstable fragments. Conclusion: The chosen 2-stage classification in MRI is well suitable for the planning of stage-related therapy.
